With anexperienced Lead Clinical Pharmacist, with over 30 years of experience in bothprivate and multiple community pharmacy, including over 8 years in Primary CareClinical Pharmacy, we provide empathic support, guidance and trainingopportunities.

Main duties of the job
Develop andmaintain good working relationships with all relevant staff involved inproviding healthcare across Southern Derbyshire.

Lead delegated areaswithin the delivery of GP clinical pharmacy by managing and developing andoverseeing systems and processes to deliver the service.
Maintainregistration as a pharmacist and comply with professional codes using owncaseload to maintain pharmacy competence
Maintain personalindemnity insurance including vicarious liability cover at the practice
Plan and organiseagreed practice workload with the practice
Comply with allorganisational and statutory requirements ( health and safety, equaltreatment and diversity, confidentiality and clinical governance)
Undertake anyother duties commensurate with the post holders grade and experience as agreedwith the GP practice.

To provide expertise in the delivery of clinical pharmacy support to the practice.
The post holder is a pharmacist who holds an independent prescribing qualification & who acts within their professional boundaries, leading and working alongside a team of pharmacists in general practice. In this role they will be supported by a clinical pharmacist manager who will support, develop, manage and mentor them.
The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews include pathology to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice.
The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage aspects of the quality and outcomes framework and enhanced services.
The post holder will help ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

3. Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences ( patients)
4. Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions

6. Able to obtain and analyse complex technical information
7. Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
8. Able to work under pressure and to meet deadlines
9. Produce timely and informative reports

13. Minimum of 2 years post qualification experience
14. In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
