Job summary
An exciting opportunity hasarisen to join Norfolk Primary Care, working as a pharmacist within a PrimaryCare Network in Norwich. You will be based at Roundwell Medical Centre withinWest Norwich Primary Care Network.
We are lookingfor an experienced, motivated, and reliable clinical pharmacist to work closelywith the practice team and other PCN staff to provide and maintain highstandards of care for patients.
As part of their employment, the postholder willachieve qualification from an accredited training pathway, includingindependent prescribing, equipping the clinical pharmacist to be able topractise and prescribe safely and effectively in a primary care setting. Thesupervision and mentorship network will be provided to support the clinicalpharmacist.
Main duties of the job
We are looking for a clinical pharmacist who willbe an integral part of the general practice team as well as part of a widerpharmacist network. They will improve patients health outcomes and theefficiency of the Primary Care team by providing direct, accessible, and timelymedicines expertise. They will work to optimise medication issues to improvepatient care, safety, and support clinical staff in the management of patients.They will need to have the ability to influence and communicate effectively withclinical colleagues, patience, and support staff.

Thepost holder is a pharmacist, who acts within their professional boundaries,working alongside a team of pharmacists in general practice and within thePrimary Care Network. They will improve patients health outcomes and theefficiency of the Primary Care team by providing direct, accessible and timelymedicines expertise including face to face contact with patients. They will work to optimise medication issuesto improve patient care, safety and support clinical staff in the management ofpatients.
The post holder will be an integral part of theGeneral Practice team, working in one or more practices across the Primary CareNetwork, as well as part of a wider pharmacist network and multidisciplinaryteam. This includes being a conduit of patient medicines information intoand out of Acute General Hospitals.
As part of their employment, the post holder willachieve qualification from an accredited training pathway,including independent prescribing, equipping the clinicalpharmacist to be able to practice and prescribe safely and effectively in aprimary care setting. The supervision and mentorship network willbe provided to support the Clinical Pharmacist.
