Hospital at Home Frailty ward have an exciting opportunity for a practitioner with advanced skills to join the team. We have 2 full time posts available, one set within Mendip area and the second one in South Somerset Area.

Our service covers the urban and rural areas of Somerset, predominantly based in one area; however, cross cover, especially at weekends, will be required.
Hospital at Home operates between 8am and 8pm; we offer a variety of shifts within our rota.
We offer flexible working solutions where possible to ensure a work-life balance. Weekend cover is required as part of the role. 30 hours is the minimum contract hours for this role.

Advanced skills such as non-medical prescribing are essential for this role.
You'll need to be a car driver with your own vehicle to visit patients. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.
Main duties of the job
Hospital at Home enables people to receive acute care and treatments in home surroundings with support from a team of health and care professionals.
* Attend MDT and huddles to discuss patient caseload and plan intervention.
* Carry out autonomous assessment and treatment plans for patients within the caseload, managing expectations of patients.
* Work within scope of practice and seek opportunities for learning from others within the team.
* Open and honest communication with patients, family, and stakeholders.
* Prioritise own time and support the team to ensure caseload is managed.
* Build relationships with neighbourhood teams to support patients in remaining within their own home.
* Work with internal and external teams to promote Hospital at Home.
* Participate in service development, identify own needs, and support training for the team and other teams in the directorate.
* Ensure as an ACP you are meeting the 4 pillars of the role.

Job responsibilities
Hospital at Home enables people to receive acute care and treatments in home surroundings with support from a team of health and care professionals.
* To be responsible for the rapid assessment, diagnostics, and treatment of individuals within the Hospital at Home service using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience. This includes highly complex decision-making across a broad range of differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and diagnoses.
* To plan and manage complete episodes of care. This will involve working in collaboration with senior medical colleagues and others when required, delegating and referring as appropriate to ensure timely, effective management to optimise health outcomes, in line with the evidence base and nationally recognised best practice.
* To play a key operational role and maintain a proactive approach to managing, updating, and leading clinical pathways.
* To promote and implement service and policy development and redesign, impacting beyond own area and other disciplines, informed by current best practice and evidence base, in accordance with values-based care.

Person Specification
Qualifications
* Professional Registration with either NMC, HCPC
* Evidence of MSc level education. Such as:
* MSc Advanced Practice.
* Equivalent other MSc mapped to the four pillars of advanced practice.
* Leadership management course or validated portfolio evidence (if not part of MSc level qualification)
* Evidence of Continuing Professional Development
* Completed ACP training within relevant clinical field
* Non-Medical Prescribing if appropriate to registered profession role.
* Demonstrated competence at an advanced practice level through Royal College or HEE Credentialing
* Relevant role-specific qualifications (i.e. Advanced Resuscitation)
Experience
* Significant experience of working as a registered practitioner in a provider organisation (e.g. acute sector, community provider, third sector, social care or primary care) and within a relevant specialty (suggested 4-5 years minimum)
* Evidence of recent teaching, mentorship, and supervision of staff
* Evidence of recent audit and/or research experience
* Experience of multi-professional working
* Ability to interpret and evaluate national and international guidelines and apply to local protocols and practice as appropriate
* Able to use high-level problem-solving and analytical skills and manage complexity
* Effective time-management and prioritisation skills
* Evidence of theoretical and practical skills allowing autonomous patient management (i.e. patient assessment, diagnosis, and management) related to clinical scope of practice
* Understand, encourage and support clinical supervision and mentorship both within and across professional roles
* Effective leadership skills relevant to role (i.e. demonstrated ability to provide clinical professional leadership)
