The Spinney Surgery is looking for an enthusiastic practice nurse to joinour friendly, hardworking Nursing Team. The Spinney Surgery has approximately patients from wide variety of backgrounds all with differing needs. Ourpractice nurses have a varied and interesting workload, dealing with patientsface to face, via telephone or occasionally video consultation. We are keen tohear from those seeking both full and part-time hours for this role.

JOB DESCRIPTION
To provide and maintain high standard of nursing care, in conjunction withnursing team colleagues, to the practice population .
To focus in the delivery of evidence based practice for patientspresenting with long term conditions and the provision of preventative healthcare to the practice population.
To provide nursing care to practice patients to include primary caretreatment, preventative care, screening and education.
DIMENSIONS:
The post holder will be required to take an active role to supportefficient and effective service delivery to the practice population of approx.1,100 patients.
To take an active role in supporting the practice to maximise achievementagainst local and national targets, QOF and Enhanced Services.
To attend relevant practice meetings.
KEY RESULT AREAS:
To provide assessment, screening treatment services and health educationadvice.
To provide assessment, diagnosis and treatment of any specific clinicalspecialism/pathway as advertised for the post (eg asthma diabetes ect.)
To work as an integral part of the practice nursing team in organising theprovision of nursing services (where applicable) for the practice.
Provide nursing treatments for patients.
Advise patients on minor ailments with referral to GP as necessary.
Provide general wound care.
Participate in national screening work such as cervical cytology anddelivery of national immunisation and vaccine schedules.
Provision of effective chronic disease management programmes to thepractice population according to agreed protocols.
Undertake the collection of pathological specimens including,venepuncture, swabs, smears ect.
Perform investigative procedures ECG and spirometry
Provide family planning and sexual health screening and advice.
ADMINISTRATIVE AND PROFESSIONAL RESPONSIBILITIES
Participate in the administrative and professional responsibilities of thepractice team.
Ensure accurate notes of all consultations and treatments are recordedcontepemporaneously in the patients computerised record and where appropriate,in their written notes using System one.
Ensure accurate completion of all necessary documentation associated withpatient care and registration with the practice.
Ensure collection and maintenance of statistical information required forregular and ad hoc reports.
TREATMENT ROOM SUPPLIES AND EQUIPMENT
Clean relevant equipment and maintain supplies of appropriate consumablesin accordance with the Practice Infection Control Policy.
LIAISON
Maintain effective communication and co-ordination with other members ofthe practice nursing team.
Maintain effective liaison with other agencies and staff concerned withpatient care and all other disciplines within the practice, with appropriateregard to confidentiality.
PROFESSIONAL DEVELOPMENT
Attend and participate in meetings with other members of the practiceteam.
Maintain continued education by attendance at courses and study days asdeemed useful. and necessary for professional development or the work beingundertaken by the practice.
To undertake an annual review in according with the Practice PerformanceManagement System.
To maintain any status/registrations required to undertake the duties ofthe role ( revalidation)
To ensure all mandatory training is kept up to date.
INFECTION CONTROL
Awareness of national standards of infection control and cleanliness andregulatory/contractual/professional requirements, and good practice guidelines.
Responsible for the correct and safe management of the specimens process.including collection, labelling, handling, use of correct and clean containers,storage and transport arrangements.
Management and maintenance of Personal Protective Equipment (PPE) for thepractice including provision, ordering, availability and on-going correct usageby staff.
HAND HYGIENE
Making effective use of training to update knowledge and skills, andinitiate and manage the training of others across the full range of infectioncontrol and patient processes.
Monitoring practice facilities and equipment in relation to infectioncontrol, ensuring that provision of hand cleansing facilities, wipes aresufficient to ensure a good clinical working environment. Lack of facilities tobe escalated as appropriate.
Safe management of sharps procedures including training, use, storage anddisposal.
Use appropriate infection control procedures, maintaining work areas in atidy, clean and sterile and safe way, free from hazards. Initiation ofremedial/corrective action where needed or escalation to responsiblemanagement.
Undertaking periodic IPC training.
Waste management including collection, handling, segregation, containermanagement, storage and collection.
Spillage control procedures, management and training Decontaminationcontrol procedures, management and training, and equipment maintenance.
Person Specification
Qualifications
Essential
1. Registered nurse with sufficient demonstrable experience post qualification.
2. GCSE GRADE A- C in English and Maths.

3. Experience working in a busy Primary Care Setting.
4. Experience of chronic disease management within general practice.
5. Ability to identify own learning needs and those of other members of the practice nurse team.
6. Ability to crucially review and communicate clinical information to both patients and health care professionals.
7. Knowledge of Patient Group Directives and associated policy.
8. Sound knowledge and experience in using of clinical system eg System 1.
9. Confident, positive attitude toward the responsibilities of the role.
