RMN – Nurse Assessor
£40 – 45k – office hours at home then travel for face to face assessments
Monday to Friday 9am to 5pm
Key Responsibilities of the RMN – Nurse Assessor
1. Completing desk-based and face to face assessments of referred people for support.
2. Ensuring an APIE approach - Assessing, planning, implementing and evaluating.
3. To review reports and evidence outcome measures to inform clinical practice, business performance and quality measures. To maintain continuous professional development and be able to produce your CPD folder upon request.
4. Able to identify and mitigate complex risks for a community setting.
5. To review pre-admission assessment in a timely and professional manner.
6. Producing clinically informed assessments with robust risk assessment and identifying risk mitigation measures.
7. Identify measures to ensure people can be enable through positive support.
8. Attend internal and external MDT meetings to further informed assessment and risk mitigation.
9. Building professional working relationships.
10. Effective time management.
11. Record keeping, ensuring that accurate safe and contemporaneous records are maintained. To effectively work alongside stakeholders and other external professionals
Essential Skills and Experience of the RMN – Nurse Assessor
12. Registered RMN / RNLD / OT
13. Experience of assessing and working with people with a variety of complex needs around their mental health, learning disability, ABI, personality disorder, autism and forensic backgrounds.
14. Understanding of continuing healthcare or transition of individuals moving from hospital/residential settings into the community and a collaborative, solutions-focussed approach to any issues associated with this
15. Applied knowledge of the Mental Health Act, CTO’s and the Mental Capacity Act, Deprivation of Liberty Safeguards and Best Interest Decision making
16. An expert in assessment and management of risk you will be an innovative thinker and natural communicator with strong report writing skills and computer literacy and an ability to work under pressure.
17. Willingness to travel to conduct face to face assessments in an agreed geographical area
18. Full UK Driving Licence and access to a vehicle
