You will undertake a key role in the development/amendment of support plans to deliver assessed needs of service users within the Short Term Assessment Review Team.
Be proactive in support planning, monitoring service users target outcomes to ensure they are appropriate and up to date to achieve goals, risk assessments, management plans and case notes/communication sheets.
Deliver support which enables carers to continue caring and empower the enablement of service users. Support personal care tasks including washing, bathing, maintaining personal hygiene, continence care, nutrition and fluid intake. Promote mobility and independence to support delivery of enablement targets.
Pushing and pulling of equipment such as hoists, wheelchairs and bathing chairs is an integral part of the post.
You will work in a locality but cover full Fife area if required.
A car driver /access to your own car would be desirable due to the large geographical area to cover our SU's

The Person
1. You must hold or be willing to achieve the Social Services and Healthcare SCQF Level 6 known as SVQ2 Social Services or equivalent to meet SSSC registration within the first period of registration.
2. You should be able to provide a regular and effective service, work to shift patterns, undertake physical requirements associated with the job and work under minimal supervision. Key skills include kindness, compassion, team working, maturity, adaptability and caring skills.
3. You will follow instructions in a safe and responsible manner, have an awareness of Adult Support and Protection and National Care Standards. You should have experience responding to situations in a calm and flexible way.
4. Person must attend a mandatory Moving and Handling course, provided by Fife Council.
As this role requires SSSC registration, the successful applicant will have three months to apply for registration.

Before confirming your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme.
