Since 2021, Occupational Therapy has played a key role within the multi-professional GP led teams of the Upper Valleys Primary Care Cluster. With continued funding for the next financial year secured (until 31st March 2025), we are excited to advertise this post to both maintain and grow the valuable role and offer of Occupational Therapy for this population by providing quality, timely, consistent assessment and outcomes for patients and their carers in the community.
Upper Valleys Primary Care Cluster consists of 4 GP Practices with whom the successful candidate will be working alongside: Dulais Valley PCC, Pontardawe PCC, Amman Tawe Partnership and Vale of Neath Practice.
Main duties of the job
The successful candidate will have broad clinical experience and an ability to adapt current skills to a varied population, knowledge of outcome measures and service development tools, experience of implementing these in practice and presenting results to a wide audience. The post holder requires well developed communication skills and have proven experience of working effectively as part of a multi professional team. They must be self motivated, enthusiastic and have a drive to maintain evidence based, quality patient care closer to home. Main duties include:
1. To be a clinical leader for Occupational Therapy within the Primary Care Cluster service, leading by example, demonstrating good practice, openness and responsiveness to emerging evidence.
2. Manage a defined caseload of clients with complex needs using evidence based /client centred principles, to assess, plan, implement and evaluate the outcomes of intervention in the community whilst maintaining associated records.
3. To provide highly developed expertise within the MDT setting of the Primary Care Cluster
4. To provide clinical leadership for junior staff as delegated by line manager.
5. To participate in the planning, development and evaluation of Occupational Therapy Services within the Primary Care Cluster, holding responsibility for defined projects.
6. To contribute to the maintenance and development of the Health Board Occupational Therapy Service.

Qualifications
Essential
7. Degree in Occupational Therapy (or equivalent OT qualification)
8. Professional Registration with HCPC
9. Accredited fieldwork supervisor for the education of OT students
10. Specific recognised postgraduate training/courses relevant to specialist area equivalent to masters level

11. Leadership / management training
12. Membership of professional body BAOT
Experience and Knowledge
Essential
13. Experience of autonomous caseload management, comprising assessment, planning intervention and evaluation within relevant clinical area
14. Understanding of the Occupational Therapy process and ability to apply current best practice to designated client group
15. Ability to interpret the results of functional assessments and activity analysis with an emphasis on skills required with designated client group
16. Knowledge of the principles of Clinical Governance and the ability to apply to working practice.
17. An understanding of the influence of health and social care policy on occupational therapy and the wider economic and socio-political issues that impact on service delivery
18. Experience of multi agency working
19. Awareness of Health and Safety legislation
20. Documented evidence of CPD
