Short Term Community Rehabilitation – Occupational Therapy Assistant Practitioner (OTAP) – Stewartry Home Teams
Dumfries and Galloway Health and Social Care Partnership (HSCP) is undergoing a period of transformative change to develop multi disciplinary integrated teams.
The Home Teams will deliver person centred care with the promotion of independence at the core of service delivery. The rehabilitation and enablement pathway is core to this development. This will ensure people and their Carers/family who could benefit from participating in the right to recovery and rehabilitation can experience excellent outcomes.
Occupational Therapy Assistant Practitioners (OTAP) are core members of the rehabilitation and enablement teams working collaboratively with Service Users, Carers/family, Health and Social Care Support Workers, Occupational Therapists, Nurses, Physiotherapists and the wider Home Teams. This role is vital in empowering and delivering better outcomes for the people who use services, enabling individuals living at home to regain independent living skills and to achieve what matters to them. The OTAP, working within the short term reablement team, has highly developed observation, communication and motivational skills. As part of the team they are supported with excellent training to reinforce techniques and approaches as part of person-centred therapeutic goals and plans.
