An exciting opportunity has arisen for experienced and enthusiastic pharmacists to join our friendly and forward-thinking team based within the Royal Primary Care Chesterfield / North Derbyshire PCN. We are seeking pharmacists who are highly motivated, innovative, and keen to develop clinical skills as part of the multi-disciplinary team.
Applications from pharmacists with experience in primary care are welcomed especially form those who have completed the CPPE pathway training (although this is not essential).
This role is purely base in primary care but as a part of Chesterfield Royal Hospital, RPC is uniquely placed to develop strong links between primary and secondary care pharmacy roles.
Main duties of the job
You will undertake a clinical pharmacy role within North Derbyshire PCN practices. This will involve a wide variety of tasks that will allow enthusiastic individuals the opportunity to develop their skills. The role will involve independent prescribing and as a PCN role structured medication reviews.
You will also be expected to undertake a senor role within the team in the provision of medicines governance advice or education and training needs.

Job description
Job responsibilities
1. Provide expertise in clinical medicines review and address the public health and social needs of patients within ND PCN member practices
2. Provide expertise in clinical Structured Medication Reviews (SMR's) and address the public health and social needs of patients within ND PCN member practices
3. Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need following discharge
4. Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related issues
5. Liaise with community and hospital pharmacy colleagues and develop referral processes between primary care professionals
6. Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates
7. Manage patients and health care professionals medicine queries
8. Increase quality and safety of prescribing through mechanisms such as audit and PDSA cycles
9. Implement drug withdrawals and alerts MHRA aimed at improving medicines safety
10. Work with primary care professional and patients to implement NICE and other evidence-based guidelines
11. Run clinics where medicines are the main interventions
12. Contribute to multi-morbidity reviews and multidisciplinary reviews
13. Provide medicines information and training to Practice healthcare professionals and admin staff
14. Act as a source of medicines information for all of the practice team and patients ( around doses, side effects, adverse events, possible alternatives around out of stocks
15. Working within the practice-based team across the ND PCN member practices to undertake medication reviews particularly in high risk groups such as

17. Problematic polypharmacy

21. Patients on high-risk medicines

24. Improve prescribing practice through educational support for all prescribers within the ND PCN Member Practice's
25. Lead on where changes in evidence require changes in prescribing across patient population where a drug is withdrawn or indications change
26. Liaise with colleagues in community pharmacy to align support for medicines adherence
27. Support improvements in clinical care through practice-based audit and implementing change
28. Reduce problematic polypharmacy and wasteful prescribing through clinical medication review
29. To work alongside Primary Care colleagues supporting vaccination programme
