Community - Urgent Community Response
Band 7 Clinical Nurse Post
The unplanned urgent community response model of care is well established within Walsall with a fully integrated domiciliary service and excellent relationships with Social Services and the Acute Sector.
The post holder will be expected to co-ordinate care and navigate the patient through pathways across the primary, secondary care interface, including the prevention of unnecessary admissions through a 2 hour national targeted NOTE, WE MAY CLOSE THIS VACANCY EARLY IF WE RECEIVE HIGH NUMBERS OF APPLICATIONS.
Main duties of the job
An opportunity has arisen to develop your role as an urgent community response nurse meeting demands of the New Ambition for Older People-Standard 7 high level of performance indicators preventing hospital admission and expediting hospital discharge from front line service areas.
The post holder will work as part of one of our unplanned, front door services and proactively manage a caseload of patients requiring complex case management in order to prevent hospital admissions and improve patient health outcomes for a limited period of time.Working autonomously, the post holder will have continuing responsibility for proactively monitoring and managing patients on the caseload and will be required to support and collaborate with the wider nursing team with regards to the community caseload.

Walsall Manor houses the full range of district general hospital services under one roof. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children's Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. This is scheduled for completion this year.

Job responsibilities
You will be part of a Multi-disciplinary, multi- Agency Team and have responsibility for a varied caseload of clients in their own homes across the borough, working under the direction of the Advanced Clinical Practitioner Lead within the UCR Team. You will be working predominantly with Older Peoples Services to develop and support this client group to remain in their homes with enhanced services. You will need to work flexibly to meet the demands of the service across 7 days and be able to work effectively with partnership agencies such as the acute, WMAS and GPS.
