This is an exciting opportunity for a highly motivated, enthusiastic and experienced nurse to join our community based team of Heart Failure Specialist Nurses delivering advanced practice care to patients with chronic heart failure in the Harrogate and Rural District Area.
The Band 7 post holder must have at least 18 months experience of managing chronic heart failure patients and have completed a Heart Failure module. They will also be an Independent Prescriber.
Consideration will be given to candidates who are willing to undertake these courses and gain experience who would join our team in a developmental role as a Band 6 HFSN with a clear competency framework to work towards Band 7
Main duties of the job
The post holder will manage an identified group of patients with complex health and socialcare needs, using a case management model. Combining highly specialist nursing knowledge and advanced skill to proactively manage patients with Heart Failure, to improve the quality, experience and choice about their care enabling them to remain at home longer.
