Job summary
We are looking for a Practice Nurse with leadership skills to join our friendly, semi-rural practice on the edge of the beautiful Yorkshire Wolds.
If you are passionate about nursing, managing long term conditions as well as keeping your hand in with treatment room practice; whilst supporting a small team of nurses and HCA's this could be the position you have been looking for!
You would work autonomously but be supported by and synergistically work with the Practice Manager and the Partners on the day to day work as well as the longer term vision for healthcare.
Main duties of the job
The primary function of the role is to provide reviews for long term conditions such as diabetes, asthma, CHD to our patients, it is essential that you are qualified in these areas. There will be an element of treatment room consultations such as depo injections, cytology and immunisations.
The secondary element is to be the head of the nursing department, providing leadership, direction and mentoring to the team; and close liaison with with Practice Manager for effective operational elements and the Partners for clinical direction/support.
We offer Extended Access appointments on a Monday evening, and there is an expectation that you will be involved with this to ensure that the contracted hours are met; as well as taking part in the seasonal vaccination clinics on weekends and house visits for housebound and care-homes.

We currently look after approx 14700 patients across Driffield, Nafferton and surrounding areas.
Our building was purpose made in 2011 and is bright, airy, modern and accessible over two floors with our own car park.
We opt into the ethical innovation and are an AccuRX ambassador as well as being involved in clinical trials.

MAJORDUTIES/RESPONSIBILITIES:
ClinicalNursing Practice
Thepost holder will be a registered nurse, who acting within their professionalboundaries, will provide high quality care for patients. They will provideleadership, mentorship and support to the nursing team. They will also supportthe Practice Manager and Partners in developing and delivering nursingservices.
Acting within their competencies the role holder will:
Undertaketreatment room duties (dressings, injections, SMEARS etc)
UndertakeLong Term Condition Reviews and management (for example Asthma)
UndertakeImmunisations and Vaccinations
UndertakePhlebotomy
Undertakeaudit and Quality Improvement work

Theindividual will work within the NMC Code of Conduct to agreed practicestandards and protocols.

1. ASSESSMENT,SCREENING AND HEALTH EDUCATION
a) Provide professionally autonomous decisions for evidence-basednursing treatments to patients - in participation with general practitioners orindependently - to agreed protocols.
b) Cover general treatment room responsibilities, includingassistance with minor surgery.
c) Provide general and specific health screening to the practicepatients (within agreed protocols, with referral to general practitioners asnecessary).
d) Administer child and adult immunisations and vaccinations inaccordance with national and local programmes. Be competent in anaphylaxis andresuscitation techniques. Ensuring safe storage, rotation and disposals ofvaccines and drugs and that adequate stock levels are maintained.
e) Administer and give advice to patients requiring travelimmunisations according to Patient Group Directions.
f) Advise patients on general health care and minor ailments withreferral to general practitioners as necessary.
g) Deliver specialist primary nurse led service, including chronicdisease management (including for all areas of QOF, the requirements of theUniversal offer and the standard PCN direct enhanced service).
h) Make appropriate referrals to other members of the primary,community and secondary care teams.
i)Promote health and wellbeing,giving general education advice on diet, contraception, smoking chronic diseasemanagement, exercise etc
j)To work only in accordance withthe UKCC Code of Conduct.
k) Where the post holder is an independent prescriber: to ensuresafe, effective and appropriate medication as defined by current legislativeframework.
l)Maintain accurate recordsaccording to NMC rules.
m) Ensure all data protection requirements are met when gathering,recording and storing patient data.
2. PATHOLOGICALSPECIMENS AND INVESTIGATORY PROCEDURES
a. Undertakecollection of pathological specimens including intravenous blood samples,swabs, smears etc.
b. Perform anyinvestigatory procedures requested by the general practitioners, or nursepractitioner. In some circumstances instigate investigatory procedures for exampleblood glucose monitoring, using own discretion.
3. CHRONIC DISEASEMANAGEMENT/LONG TERM CONDITIONS
Diabetes Mellitus
This role requires completion of asuitable accredited course; diploma or degree
a. Initiates the process of diagnosis on patients with suspecteddiabetes.
b. Manages patients newly diagnosed and ongoing diabetes usingpractice template protocols/NICE Guidance and local guidelines.
c. Undertakes annual review of patients understanding and ability toself manage.
d. Recognises areas of disease management that may be improved byaltering medication and refers to GP as appropriate.
e. Refers to GP, dietician, chiropodist and diabetes and specialistnurses as necessary
f.Maintains a disease register.
g. Audits outcomes of care against standards and NSF for Diabetes andinitiates change as necessary.
Respiratory Care
This requires accredited training; Diploma level
a.Manages newly diagnosed patients with asthma.
b.Maintains a disease register.
c.Works within agreed local/BTS guidelines and patient groupdirections to manage patients with asthma, aiding diagnosis, reviewing inhalertechnique and advising on treatment changes.
Cardiovascular Disease
a.Develop and maintain practice register for secondary prevention ofCHD.
b.Works with patients to set action plans for changes of lifestyle.
c.Works within agreed protocols and templates when managing riskfactors to enable patients to manage own disease.
d.Provides psychological support by discussing interventions andtreatments.
e.Recognises areas of disease management that may be improved byaltering medication and refers to GP as appropriate.
4. Clinical Leadershipand Development
a.Work closely with the Partners and Practice Manager to support thePractice strategy and forward view.
b.Provide effective cohesive Team Leadership to the Nursing and HCAteam members, including development, advice and support.
c.Participate in clinical supervision both as supervisor andsupervisee.
d.Monitor the effectiveness or their own clinical practice throughquality assurance strategies such as the use of peer audit and review.
e.To delegate clinical responsibilities appropriately (ensuring safepractice and the task is within the scope of practice of the individual).
f.Ensure awareness of responsibilities in relation to ClinicalGovernance, health & safety and infection control.
g.Maintain professional registration.
h.Work within the latest NMC Code of Professional Conduct.
i.Work collaboratively with colleagues both internally andexternally, promoting effectivecommunication, relationships and team working within the practice.
