We are looking for an adult trained nurse working at an advanced level who is looking for an new challenge working alongside our care navigator and clinical operational lead to support our online consultations and our new neighbourhood team.
Main duties of the job
This will be a new role supporting the development of our new neighbourhood team for the PCN. As a PCN we are required to carry out holistic assessments to cover all health, social and wellbeing aspects of the patients in care homes to develop personalised care and support plans. You will be required to identify the complex care needs of those frail patients and support the care home staff using risk stratification tools to identify this cohort of patients like the Rockwood score and electronic frailty index. You will visit care homes regularly building a rapport with the staff and patients to support the nursing team with assessment of the patients needs, liaise with the wider MDT's, do referrals to appropriate others as needed and liaise with the named GP as needed. This role will also provide holiday/sickness cover the electronic e-hub consultations online for the Practices. You will advise/prescribe using an electronic consultations system with EMIS to patients within a remit of minor injuries and illness, full training will be given.

You will work as an autonomous practitioner evaluating patientswith complex care needs by using physical assessment skills, independent nurseprescribing (V300) Assess, create or update personalised care plans forvulnerable patients living out local primary care network area of Rye, Camber,Winchelsea Sedlescombe, Westfield Northiam, Broad oak Robertsbridge and BurwashTriage and manage acute and non- complex medical needs ofpatients who are housebound, palliative care and vulnerable, within your scopeof practice and knowledge. Where other clinical support is, needed refer to GPor seek appropriate support of other community basis conjunction with our care coordinators, work with other localservice providers such as GP surgeries, community services, secondary care andvoluntary sector to ensure that there is good communication between themulti-disciplinary patients presenting with a range of acute and chronicmedical conditions, providing subject matter expert advice or support patientsto access care for these issuesImplement and evaluate individual specialised treatment plansfor chronic disease patientsIdentify, manage and support patients at risk of developinglong-term conditions with the aim of preventing and managing the adverseeffects these diseases may have on the patients healthProvide advanced, specialist nursing care to patients asrequired in accordance with evidence based evidence, NICE and the NSFSupport the undertaking of simple diagnostic procedures such:-Undertaking of ECGs and blood pressure monitoringUndertaking observations including monitoring for sepsisUndertake the collection of pathological specimensRequest pathology services as necessaryProcess and interpret pathology and other test results asrequiredMaintain accurate clinical records in conjunction with NHSguidelines for good practiceYou will be at the heart of our primary care network program tosupport vulnerable patients. Such as those who have complexmedical issues, palliative care patients, those who have Learning disabilities,Autism, Serious mental health illness and those who struggle to access medicalservices directly from a GP practice and would benefit from support aroundmanaging their ongoing care. You will visit patients in their own homes and incare homes, review and assess their health and wellbeing needs, design andagree with the patients a personalised care plan. Then work with patients,their families, carers and the wider Multi-disciplinary team within communityand secondary care to ensure that they can live the best possibly life withinthe boundaries of their medical limitations. Our aim is for you to workalongside key community services already in place to provide co-ordinatedpersonalised care for those in care homes/housebound patients to preventun-necessary hospital will also offer support to our established e-hub for online consultations providing cover when needed for sickness/annual leave. You will be able to assess and manage patients presenting with a range of acute and chronic medical conditions such as acne, rashes, eczema, contraception, HRT, conjunctavitis, sore throats, urine infections, pain management, colds and respiratory issues.
1. Recognise, assess and refer patients presenting with mental health needs.
2. Support patients in the use of prescribed medicines or over the counter medicines (within a scope of practice, reviewing annually as required)

Essential
5. Advanced Practice Qualification (Masters Level) or equivalent experience (BSc Nurse Practitioner/Community Specialist Practitioner) and have completed a level 7 module which will give you access to the e-portfolio route of registration as an Advanced Nurse Practitioner (Health Education England requirement) which is an expectation of this role, Experience of working in a primary care /Urgent Care/A&E Experience of working as a GP practice or relevant similar experience. Experience of carrying out through medical assessment of patients and creating treatment plan of their needs Experience of prescribing and undertaking medication reviews or similar relevant experience.
6. Qualified Independent Nurse Prescriber V300 essential for this role.

7. Qualified Triage assessment practitioner or other relevant experience Minor Illness Qualification- or willingness to undertake training Mentoring and supervision training undertaken.
