Pertemps Social Care are recruiting social workers for roles within a Hospital Team in Luton.
You will be working as part of a multi-disciplinary team, assessing and facilitating safe and timely discharges with a commitment to promoting independence. You will work in a supportive team of Social workers and Social care support officers. The team works to tight deadlines, with a fast turnaround of work.
Your core responsibilities will include:
1. Rapid Assessment under Discharge to Assess guidelines for hospital discharge
2. Assessing service users, mental capacity and making best interest decisions
3. Liaising with the discharge liaison team, as well as ward staff and family members.
4. Arranging care packages on discharge and step down beds.
5. Completing reviews and support plans.
6. Reporting any delays with service users.
