An exciting and unique opportunity has arisen to be a part of the Mental Health Intensive Support Service based in East Cheshire.
This service provides specialist assessment, treatment, interventions & support, to help people recover from complex mental health problems & to regain the skills & confidence to live successfully in the community.
As a Senior Support Worker, you will be expected to support the MDT in providing intensive support to a small cohort of patients, providing high quality levels of care in order to assist these patients to live fulfilled and independent lives.
You will be working alongside the Primary Worker and Occupational Therapist to ensure effective and adaptive patient care with a strong focus on recovery, identifying relapse indicators and promoting independence.
A genuine attitude of care and dedication to this type of cohort of patients is necessary; candidates with lived experience are encouraged to apply.
Main duties of the job
You will be responsible for ensuring all patients engage in the agreed care plan and assist in encouraging inclusion in local community services, to promote daily living skills. An effective therapeutic relationship with patients is pertinent to contribute, plan and monitor interventions and treatments, under the supervision of the MDT.
You will be responsible alongside the MDT for active participation in training of Care Staff/third party organisations that will be in place in the community that support the patient, such as Supported Living staff or Care Agency staff.
The MHIST team will be proactive in pre-empting crisis and preventing admission into hospital, and therefore you must be able to demonstrate excellent skills in risk management, adaptive care planning and communication skills across a range of scenarios.

Although this role is based in East Cheshire flexibility is required to support a Trust wide service.

Qualifications

1. Recent relevant experience of working to support and integrate people with mental health issues towards recovery
2. Experience of working within a Mental Health community setting, rehab setting or equivalent.
3. NVQ Level 3 in Health & Social Care or Equivalent
