We are looking for a motivated Physiotherapist with relevant skills and experience to join the Discharge to Assess South Team on a secondment basis for 12 months. An opportunity to work in a supportive team, supporting a busy, productive service as part of the Manchester Community Response Teams.
The Discharge to Assess Team supports timely assessment of patients at home at the point of discharge to identify health and social care needs that can be delivered in the community for up to 7 days. All referrals are received from Hospital Wards. The Discharge to Assess Team currently works from 8am until 8pm 7 days per week.
The team consists of physiotherapists, occupational therapists, nurses, healthcare support workers and administrative support to enable an efficient assessment and review service for all patients referred into the service. The Discharge to Assess team also work closely with reablement and social services to ensure effective care needs are met by the right people, at the right time.
Please be aware you must hold a valid UK driving licence and have daily access to a car to apply for this role.

Essential
* Degree/Diploma in Physiotherapy
* HCPC Registration
* Evidence of relevant post registration training / CPD
Desirable
* Clinical educator
* Member of CSP
Experience
Essential
* 2 years experience as a band 5 in core clinical areas
* Experience of timely assessments and developing and delivering care plans to meet these needs
* Supervision and education of less experienced staff, students or assistants ensuring safe working practices
* Experience of working collaboratively as part of a multi-disciplinary team
Desirable
* Community experience
* Previous Band 6 experience
Knowledge
Essential
* Knowledge of current health policy and relevance to the delivery of health care service.
* Knowledge and understanding of complex medical, social and psychological needs of older adults.
