A fantastic opportunity has arisen for a highly motivated and enthusiastic advanced level Pharmacist to facilitate and lead pharmacists, pharmacy technicians and foundation level pharmacists within Carlisle Primary Care Network (PCN).
We are looking for someone with extensive, up to date clinical pharmacy knowledge and experience, able to work effectively within a multidisciplinary team. The applicant will need to have excellent communication, clinical and negotiation skills, as well as experience of supervising other staff, providing education and training.
The candidate would ideally have or be working towards a non-medical prescribing qualification and have completed the Centre for Pharmacy Postgraduate Education (CPPE) Primary Care Pharmacy Education Pathway (PCPEP). They must have at least 2 years’ experience of working in primary care and be able to support GP practices to achieve optimum health outcomes for their patients. Competence in undertaking structured medication reviews, particularly in frail and elderly patients, those with complex or multiple co-morbidities, and for residents in care homes, is essential.
The successful candidate will be responsible for:
1. The leadership and proactive development of specialist clinical pharmacy services to patients within the Carlisle PCN.
2. Providing specialist clinical pharmacy services to patients within the Carlisle PCN and developing services in line with appropriate national standards.
3. The post holder will be involved in local incentive schemes, provide leadership on quality improvement and clinical audit as well as long-term condition clinics and managing some aspects of the Quality and Outcomes Framework.
4. Providing expert and highly specialised pharmaceutical information and advice to medical and nursing staff, other health professionals and patients, both within the Trust and externally as appropriate.
5. To work as an independent prescriber to ensure patient medication is optimised for each patient and to undertake structured medicines reviews with patients.
6. Providing clinical training, guidance, support and supervision of junior pharmacists, pre-registration trainees, other Pharmacy staff and other PCN staff as required.
7. Promoting the medicines optimisation agenda by leading specific audit and research projects and provide support to others.
8. Evaluating and expanding the role of pharmacy services within North Cumbria and in particular within the PCN as well as delivering the required medication safety and governance agendas.
