Main duties of the job
What can we offer you as a Practice Nurse?
1. As a Practice Nurse, we can offer you an annual salary range of £18 to £20 per hour depending on your experience, skills and qualifications.
2. up to 30 hours - flexibility for these to be spread over three or four days
3. Full support from an experienced close-knit clinical team including GPs, ANPs, PNs and HCAs
4. Enrolment to the NHS pension scheme

Job responsibilities
10. Develop, implement and embed health promotion and well-being programmes
11. Undertake the child vaccination programme
12. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
13. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
14. Provide guidance to patients in the use of prescribed and over-the-counter medications on side effects and dosages
15. Oversee the provision of wound care services to patients
16. Request pathology services as necessary
17. Process pathology results as required
18. Maintain accurate clinical records in conjunction with extant legislation
19. Ensure read codes are used effectively by all members of the nursing team
20. Develop, implement, embed and participate in well woman clinics
21. Develop, implement, embed and participate in well man clinics
22. Chaperone patients where necessary
23. Assist GPs with minor surgery when required
24. Prioritise health issues and intervene appropriately
25. Support the team in dealing with clinical emergencies
26. Recognise, assess and refer patients presenting with mental health needs
27. Implement vaccination programmes for adults and children and travel
28. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
29. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
30. Support the clinical team with all safeguarding matters, in accordance with local and national policies
31. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
32. Deliver opportunistic health promotion where appropriate
33. Managing chronic disease as appropriate

36. Develop and review audit protocols and process for the practice, effectively utilising the audit cycle
37. Develop, review and implement nursing protocols in conjunction with the partners

39. Participate in local initiatives to enhance service delivery and patient care
40. Support and participate in shared learning within the practice
41. Continually review clinical practices, responding to national policies and initiatives where appropriate
42. Participate in the review of significant and near-miss events applying a structured approach root cause analysis (RCA)
43. Coordinate the nursing team rota, ensuring sufficient staff are available to meet patient needs
44. Lead the planning and implementation of changes within the nursing team, enhancing the provision of services to patients
45. Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation
46. Act as a clinical leader in the delivery of practice nursing services to patients, ensuring that the needs of the patient are the priority
47. Lead the nursing team in the planning and implementation of local guidelines, protocols and standards, and of local projects or initiatives
48. Promote the role of the nursing team in the provision of care
49. Participate in practice team meetings, delivering the nursing agenda and run nurse department meetings
50. Promote a learning environment with a no blame culture
51. Lead the QoF strategy and delivery for LTC management, maximising income
52. Responsible for managing and maximising CCG, PHE and NHSE driven targets (childhood immunisations, flu campaigns, smears etc)
53. Support the development of others in order to maximise staff potential
54. Lead others to make realistic self-assessment of their knowledge and skills, challenging any complacency and actions that are not in the interest of the public and/or users of service
