Job summary
Our Community Services team based in the Scarborough locality is looking for band 5 Community Nurses on a fixed term basis (9 months).
This could be your opportunity to experience working in the community for the first time and we guarantee you will value the experience of the personalised care you can provide when assessing & caring for patients in their own homes, and you may wish to apply for future permanent positions should they arise.
Your planned induction would include a training programme delivered by our specialist nurses & for newly qualified staff we have a preceptorship programme. We have a clear competency framework so you know what is expected within your role & can plan development of your competencies with support from your line manager.
As a community nurse you need your own transport and to be comfortable working alone in patient homes. You will work closely with other community nurse colleagues and the wider MDT within our own services and other agencies including the acute hospitals, Hospice, General Practice, Social Services & the voluntary sector. We are a 24/7 service so there is opportunity for different types of shifts.
Main duties of the job
The main duties of the role include -
1. Holistic patient assessment, formulation and reviewing of care plans
2. Holding a non-complex patient caseload
3. Management of pressure ulcers & wounds
4. Palliative & end of life care
5. Bladder & bowel care including catheterisation and continence assessments
6. Medicines Management
7. Injections (sub cut & IM)
8. Phlebotomy
9. Provision of equipment to support care
10. Safeguarding
11. Participation in daily safety huddles & handovers
12. Line management & supervision of junior staff
13. Practice supervision for nursing students
14. Continual learning and professional development

Essential
15. Evidence of how you will efficiently transport self and equipment to/from multiple venues over a large geographical area during each shift in the UK
16. Adequate IT skills required to utilise electronic patient records, electronic communications, clinical stock ordering

18. Knowledge of the specific service area, team and role evidenced within the supporting information of application to vacancy

22. Degree/diploma in adult nursing (or in final year of nursing degree programme)
