Due to reorganisation and restructure in the integrated service, we have Band 6 opportunity for an Occupational Therapist, Physiotherapist, Social Worker or Nurse to work in our Hospital Discharge Team as a Discharge Coordinator.
Working to a home first agenda, the role is to support discharge across inpatient, emergency department and community settings.
Our service consists of Health and Social Care colleagues.
Consultants, GPs, Advanced Care Practitioners, Social Workers, Nursing, OT, Physiotherapists, Discharge Team and Multi-Disciplinary Assistants all work together to support patient flow. We operate 7 days, 365 days a year to support some of our most vulnerable, frail patients.
Main duties of the job
Due to reinvestment, an exciting opportunity has arisen within the Harrogate and Rural Alliance (HARA) and Intermediate Care (IC) Hub at Harrogate Hospital. The role is open to all AHP registered professionals and NMC Registered staff. The post is within our Discharge Team working across all wards as a Discharge Coordinator.
Taking a lead on all Complex Discharge Planning the Discharge Coordinator works alongside the MDT to support and promote patient flow. The Discharge Coordinator works clearly and passionately towards the HOME FIRST AGENDA.
We are seeking to expand the HARA / IMC Team in line with the planned further development of the Intermediate Care Hub. The ARCH team cover across acute and community setting. Our staff work alongside our NYC colleagues to provide interdisciplinary care, developing skills such as advanced practice.
We are a fully integrated community team and you will be expected to provide leadership for the junior members of the Discharge Team.
Working to a Home First Agenda, HARA / IMC Hub provides care and support 7 days a week to enhance and facilitate early hospital discharge / admission avoidance.
In conjunction with our NYC colleagues, we are a team consisting of Geriatricians, GPs, Advanced Care Practitioners, Nurses, Occupational Therapists, UCR Practitioners, Physiotherapists, Multi-Disciplinary Assistants and our District Nursing Colleagues.

4. NMC Registered or a equivalent professional qualification in nursing, social work, therapy or management in care services
5. Experience of working in complex discharge
6. Evidence of recent academic and professional studies
