Job Title: PCN Admin Care Coordinator
Job Type: Part-time 22.5 per week
Salary: Up to £15,000 per annum, depending on experience.
Location: Office based across all four sites (Bishops Waltham, Stokewood, Twyford and Wickham Surgeries) in which the successful candidate will be required to travel between.

Job Description
As an Admin Care Coordinator, you will play a pivotal role in ensuring the smooth operation of our Primary Care Network (PCN). This position involves a combination of administrative tasks, project support, and patient interaction, all designed to enhance the quality of care provided by our network. You will be a key player in helping the PCN achieve its contract requirements and targets while providing vital support to both the management team and the broader network.
Key Responsibilities
1. Conduct regular audits of patient records and administrative processes to ensure compliance with clinical standards and data protection regulations.
2. Gather and analyse patient feedback via SMS, ensuring that insights are shared with the relevant teams to drive continuous improvement.
3. Execute and manage searches on EMIS to retrieve and analyse patient data as needed for audits, reporting, and clinical decision-making.
4. Assist in the coordination and implementation of PCN projects and services, ensuring they are delivered efficiently and meet quality standards.
5. Plan and organise vaccination clinics, including scheduling, contacting patients, and ensuring all logistical aspects are in place.
6. Proficiently use EMIS, AccuRx, Microsoft Teams, Outlook, and Excel to manage day-to-day tasks, communicate with stakeholders, and maintain accurate records.
7. Provide administrative and organisational support to the PCN management team and Board, including preparing reports, organising meetings, and managing communications.
8. Meet and collaborate with external teams and organisations to support PCN initiatives, ensuring effective communication and partnership working.
9. Assist the PCN in achieving contract requirements and targets by providing necessary data, reports, and administrative support.
Skills and Qualifications
Experience (Desired):
1. Previous experience in an administrative role, preferably within a healthcare or PCN setting.
2. Knowledge of EMIS, AccuRx, Microsoft Teams, Outlook, and Excel.
3. Experience in handling patient inquiries, managing appointments, and providing support to patients, ensuring a high level of patient care and satisfaction.
4. Experience in maintaining and managing patient records, performing data searches, and conducting audits, with a strong understanding of data protection regulations.

1. Excellent verbal and written communication skills, with the ability to liaise effectively with both internal and external stakeholders.
2. Demonstrates strong listening skills to accurately understand patient needs, concerns, and feedback, ensuring that responses are empathetic and appropriate.

4. Ensures all communications, whether written or verbal, are precise, accurate, and free from errors, particularly when handling patient information and documentation.
