Due to planned growth expectations TPMP has a fantastic opportunity for a Clinical Pharmacist to join our multi-disciplinary team working across our Earby and Colne GP surgeries
Joning our team means you will be consulting with patients using face to face, telephone & via e-consultation methods. You will be responsible for the provision of medical services to our patient population managing a caseload dealing with a wide range of health needs. You will be supporting & supported by the wide range of clinical and non-clinical team within the practice offering physical examinations, investigations and case management inc referral as needed.
An active and vital part of the team you would be expected to work closely on Quality Improvement work, QOF, assisting and supporting duty doctor and must be willing to support Enhanced services and extended hours inc occasional Sat AM work by rota.

Main duties of the job
The post holder would be a pharmacist, who acts within their professional boundaries, supporting, integrating and working with the practice
This post would require both, to work as part of a multi-disciplinary team in a patient-facing role, but also individual clinical decision making around your caseload of patients.
A Key CORE responsibility will be around Signing of EPS repeat prescriptions - ensuring all monitoring and reviews are up to date. (including structured medication reviews) for chronic disease conditions, patients with complex polypharmacy and priorities for contractual services ( Direct Enhanced services ,QOF), Incentive schemes and support practices with implementation of CQC recommendations.
The post holder will be involved in supporting General Practice staff with regards to prescription and medication queries, and if necessary, to be involved with improvement of existing systems.
Where necessary, post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
You must HOLD or about to complete your independent prescribing qualification.

The team at TPMP are amazing colleagues to work with and comprises of 3 GP Partners, 9 Salaried GPs, 1 Senior Clinical Practitioner, 1 PA, 1 Pharmacist, and 16 Nursing & HCA staff, along with non-clinical team of 30+ staff managed by our Business Manager.
Our practices have been rated as Good by CQC and we take pride in delivering quality care to our patients and have a strong training and personal development ethos. We are a very active part of Pendle East Primary Care Network who we work closely.
We are using EMIS Web and Accurx as our clinical system along with Econsult to provide digital platform to our patients.
