We are recruiting forpermanent CHAPs to work alongside our Registered Nurses, ensuringoptimum levels of person centred care is delivered consistently toall residents. You will be supporting the Home Manager todeliver service levels within set budgets and providing continuedsupport to other colleagues enabling them to contribute fully tothe care team overall.
As a CareHome Assistant Practitioner you will be:
* Assisting the registered Nurse andconsistently monitoring the standard of care being delivered to allresidents
* Assisting the registered Nurse inensuring that all residents receive quality medical and personalcare
* Responsible for monitoring the wellbeingof each resident, being aware of any changes which impact on careand care plans
* Supporting the Home Manager inthe implementation of all policies, procedures and businessobjectives along with preparing staff duty rota’s
* Identify training needs of the team and support with theimplementation of the home’s training programme
* Conducting staff supervisions and appraisals ensuringthey meeting external regulator guidelines
* Acting as a focal point for contact with residents, theirrelatives and visitors, as well as health and social careprofessionals

* Passionate about providing quality care in ourhomes
* Experienced in a relevant care role andable to evidence full competence
* Qualified in,or working towards, a level three health and social carequalification
* Committed to ongoingprofessional development
* Up to date with allpersonal essential learning
