Job summary
LakenhamSurgery is currently seeking a qualified ANP or Nurse Practitioner to join ourfriendly and supportive GP, Nursing and Practice team. This key role aligns closely with both the GPand Nursing Teams, primarily supplementing practice capacity for On the Day andshort-term pre-bookable appointments but also seeing and treating patients withlong term conditions and a range of other presentations as required. The successful candidate will provide care thatencompasses history taking, holistic clinical assessment, diagnosis, treatmentand evaluation of care, demonstrating safe clinical decision making andawareness of professional competencies and boundaries at all times. Ideally, they will see, assess and treat allpatients ranging from young children to elderly members of our patient populationand have experience of dealing with all types of medical conditions, butrecognise the need, where appropriate, to consult with or refer onto colleagues.Dealing with referrals, prescriptions and test results for episodes of careprovided will also be required.
Previousexperience in a GP Practice is essential, together with minor illness and injurytraining. The successful applicant willneed excellent interpersonal and communication skills and we are looking for a reallygood team player who is able to provide support and help to colleagues toensure that we provide the best possible care to our patients, in a mutually supportive environment.

Main duties of the job
1. Manage individual patient consultations
2. Help patients to identify their health needs
3. To assess and diagnose across a wide range of health care conditions, order investigations, plan and provide treatment as needed and discharge patients, where appropriate, without reference to a GP
4. To recognise patients with more complex problems who do need the expertise of a GP and liaise appropriately with a member of the GP team, initiating appropriate and timely consultation and or referral, when a condition exceeds the Nurse Practitioners scope of practice and/or expertise
5. To demonstrate critical thinking and diagnostic reasoning skills in clinical decision making
6. To assess, tolerate and manage risk and deal with uncertainty in accordance with the context and requirements of the role
7. To prescribe in accordance with regulations and with the post holders training and registration
8. To prescribe safe, evidence based, cost-effective and appropriate medication as defined by current legislative framework and in accordance with local non-medical prescribing policy
9. To follow agreed national and local treatment guidelines, ensuring all care is delivered within the professional scope of practice.
10. To have clinical and professional accountability for prescribing practice, work at all times within clinical competence and with reference to the NMC professional standards.

11. To provide a high standard of evidence based triage and minor illness assistance to the Doctors and other members of the primary healthcare team and maintain a high standard care for our patients. This will include health promotion, treatment, screening and chronic disease management and preventative care as appropriate
12. The duties will include all tasks normally undertaken by an experienced Nurse Practitioner Prescriber in Triage & Minor Illness including history taking, clinical assessment, diagnosis, treatment and evaluation of care and in addition any tasks agreed between the post holder and the Doctors as appropriate, having regard to current training
13. To work directly and independently with our patients, diagnosing, initiating and concluding treatment including prescribing within appropriate professional scope and competencies
14. To use SystmOne and Ardens Templates for recording of consultations and clinical administration, demonstrating good practice in clinical note making
15. Adhere to the Nursing & Midwifery Council (NMC) Code of Professional Conduct and to be familiar with The Scope of Professional practice and other NMC advisory papers.

16. Nurse Practitioner qualification (including MSc)
17. Independent & Supplementary Prescriber qualifications
18. Registration with NHSBSA as a Prescriber
19. Eligibility to work as a Nurse in the UK and with UK current work permit if required
20. English language skills sufficient to enable positive and effective verbal and written communication about medical topics with patients and colleagues
21. RGN with current NMC registration
22. Ability to work as part of Enhanced Access service provision
