Band 4 - Assistant Practitioner Nursing

This substantive post is for the Mid Beds Community Nursing Team.

Main duties of the job
You will need to be a motivated and enthusiastic person, who has excellent communication, IT literate and problem solving skills, alongside a passion for patient centered care. You will work corporately within our Primary Care Team and will be supporting the Community Nurses and the wider MDT, to provide high quality care to housebound residents who live with a wide range of complex health conditions.
Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important.

The successful candidate will be expected to be flexible and assist colleagues to cover service needs, which will include working across the wider integrated team. Access to a car in order to carry out service duties is essential.
A full induction programme will be provided.

2. oGCSE Maths or equivalent
3. oDiploma level relevant qualification or equivalent experience

4. oDemonstrates appropriate emotional self-regulation and maintains professional boundaries at all times
5. oGood experience of working with community health care teams
6. oExperience of working with people on a curative and or palliative pathways
7. oExperience of with the public demonstrating good customer care skills
8. oExperience of health coaching and promoting independent living
9. oExperience of working with people with physical health requirements
10. oExperience of managing caseload of patients
11. oExperience of working with people with learning difficulties and or mental health issues

Knowledge and Skills

13. oDemonstrates good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures
14. oDemonstrates good understanding of the Mental Capacity Act / Deprivation of Liberties and applies principles to everyday practice seeking advice / guidance as required
15. oAbility to prioritise work and work in a time-managed and effective way to meet patients' appointments and demands of the role
16. oComprehensive clinical skills (, venepuncture, Doppler, wound care, continence care) etc
17. oAbility to manage a caseload of patients
18. oBasic ability to use Microsoft packages - Word, Excel - and databases.

20. oBe able to undertake frequent standing, kneeling, crouching, hoisting and other moving and handling procedures
