This is a new post for South Gwynedd for the Older Persons catchment.
Ysbyty Gwynedd is a 580 bed District General Hospital serving the population of Gwynedd, Ynys Mn as well as border areas of Gwynedd. The hospital provides a comprehensive range of medical and surgical specialties, as well as paediatrics, maternity and critical care services. The site also includes a mental health inpatient unit (Hergest).
The post holder will be responsible for Older Persons Services, covering GP surgeries in Dwyfor, South Gwynedd.
The post will provide CPA reviews and ward round for inpatients for the catchment area, outpatient clinics and medical input to the memory service. Dealing with organic referrals for the catchment area.
The post holder will provide specialist psychiatric assessment, treatment and management for people with cognitive impairment (including Young Onset Dementias) and those over 70 with functional illnesses - transition arrangements are in place for developing an age equal or needs based specialist Older Persons Mental Health Service from the catchment area) both functional and organic.
Main duties of the job
The post holder will provide specialist psychiatric assessment, treatment and management for older adults based in Dwyfor, South Gwynedd.
The post holder will be based with the Old Age Community Mental Health Team to ensure clinical leadership of the multi-disciplinary team. The post holder will assist with clinical supervision of the team. It is anticipated that the post holder will assist in training of team members in clinical matters.
The service operates with both inpatient and community management of patients. The post holder will work collaboratively with the Acute Care Services.
There is a dedicated session within the job plan for consultation with the Acute Care Service. In the event of one of the acute care consultants being on leave overall responsibility for the inpatients is held by the other acute consultant. The post holder is expected to support acute care with their acute care session.
The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.

1. Full Registration with the General Medical Council.
2. Minimum of four years (full time or equivalent) in postgraduate training in the UK or equivalent EEA training.
