Job summary
The Early Stroke Rehabilitation Team (ESRT) is a friendly, enthusiastic and supportive team who are passionate about delivering high quality patient-centred care.
We have an exciting opportunity for a band 6 physiotherapist to join the team on a fixed-term basis (12 months). You will work alongside a team of physiotherapists, occupational therapists, speech and language therapists, clinical neuropsychologist, stroke nurse and rehabilitation assistants.
The ESRT is based in Bletchley, Milton Keynes, you will be working in the community as part of the team providing holistic, goal orientated rehabilitation for people who have a new stroke.

Main duties of the job
1. Working as part of a multi disciplinary team
2. Undertaking assessment and treatment of patients with stroke in a community setting
3. Plan and implement a comprehensive programme of intervention for patients with stroke, including those with a complex presentation/multi-pathologies
4. Work alongside our partners across the stroke pathway for example the Stroke Clinical Specialist, other community services, and the acute hospitals
5. To implement strategies to empower patients and families/carers to take a lead role in their rehabilitation and long term management of their condition
6. To work collaboratively as part of an inter-disciplinary approach within ESRT and Neuro Rehab Service and Neuro Clinical Specialist Team by attending and contributing to multidisciplinary meetings, participation in patient discussions/treatment plans and discharge planning

Job responsibilities
To provide a high standard of clinical care for the patients under your management, and to be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care and that of rehabilitation assistants or students under your direct supervision.
To undertake the comprehensive assessment of patients, including those with a complex presentation/multi-pathologies; use advanced clinical reasoning skills and manual assessment techniques to provide an accurate assessment/diagnosis of their condition
To implement strategies to empower patients and families/carers to take a lead role in their rehabilitation andlong term management of their condition.
To formulate, deliver and evaluate individual physiotherapy treatment programmes based on a sound knowledge of evidence based practice and treatment options using clinical assessment, reasoning skills and knowledge of a variety treatment skills.
To work collaboratively as part of an inter-disciplinary approach within ESRT and Neuro Rehab Service by attending and contributing to multidisciplinary meetings, participation in patient discussions/treatment plans and discharge planning.

7. oFull & current UK driving licence and access to a suitable vehicle.
Education & Qualifications

8. oUK registration with Health and Care Professions Council (HCPC)
9. oPhysiotherapy degree or equivalent Physiotherapy professional qualification

10. oEvidence of post registration CPD relevant to Neurology/ Stroke
11. oMembership of ACPIN or other relevant specialist interest group
Previous experience

12. oBroad base experience working as a physiotherapist in the NHS in a community or acute setting
13. oExperience working within neurological conditions and/or stroke in a community or acute setting
14. oExperience of working effectively in a multi-disciplinary team
15. oAbility to plan, prioritise and manage own workload effectively

16. oUndertaken or participated in a service delivery project, quality improvement project or audit
17. oExperience of being a student clinical educator
Skills and knowledge

18. oExcellent communication skills including good verbal communication skills in the English language
19. oExperience with neurological assessment, treatment and use of outcome measures relevant to stroke
20. oAbility to formulate clear problem lists, treatment plans and goals
21. oConfident in IT skills, Microsoft Office and patient data recording systems
