Job summary
Brackley Medical Centre are looking for an experienced Emergency Care Practitioner/Advanced Nurse Practitioner/Advanced Paramedic (qualified prescriber preferred) to work alongside our friendly and experienced team. We are looking for a motivated and skilled person who loves the challenge of working in general practice.
Join our excellent Urgent Care Team working across a variety of areas, on the day appointments, triaging, care homes and hospital rounds. We also have specialist clinics for our ANP's supporting their interest in specific areas.
Main duties of the job
Working as part of a team, you will be providing an efficient integrated urgent care service to our patients. Working independently through mainly face to face consultations and/or home visits, you will be accountable for delivering safe, clinical decision-making and expert care to a high standard. Acting within your professional boundaries, your duties will include all (or part) of the patient journey including initial history taking; clinical assessment; diagnosis; treatment; supply of medication and evaluation of their care.
Th role is to to provide robust on the day appointments for our growing patient list and in addition our Urgent Care Team will be leading on Care Home Rounds and Hospital Rounds for the Community Hospital situated on the second floor of our building.

We are a 16k patient GP surgery which moved into a purpose built site 2 years ago, providing staff and patients with outstanding facilities, with easy access and free parking. We have a large and diverse clinical team of GP's, Clinical Practitioners, Advanced Nurse Practitioners, Practice Nurses and HCA's as well as a large Patient Services and Administration Team.
We are a member of the Brackley and Towcester Primary Care Network and have the services of Clinical Pharmacists, First Contact Physiotherapists, Social Prescribers and a Mental Health Workers.

Job responsibilities
Working in out Urgent Care Unit to:
1. Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans when working in a clinic or home visit environment.
2. Through telephone clinical assessment, obtain a relevant and sequential history of the presenting complaint to determine a working diagnosis.
3. Formulate a management plan for the patient. Use other sources of healthcare professionals (such as GP or Pharmacists) if required to support diagnosis or next course of action.
4. Recognise the need and initiate emergency treatment in order to save life or to safeguard the interests of the patient and refer to emergency services when appropriate.
5. To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence.
