This role is for Medicine Management Optimisation across the disciplines of both General Practice and Care Homes. If you are interested in working in this dynamic environment then please come and join this enthusiastic dynamic team.
As a Clinical Pharmacist in Surrey Heath you will join the growing clinical pharmacy and multi-disciplinary PCN team. Working across a nominated general practice to support the responsibilities and deliverable of the Surrey Heath Primary Care Network.
Initially, your time will be allocated on rotation across one nominated GP practices and also to the PCN Care Homes Team, to enable you to build your clinical skills and confidence and benefit from the diversity of portfolio working.
You will be responsible for supporting the practice(s) to implement effective medicines management, identifying areas for improvement, and initiating and managing change. In addition the role will be responsible for supporting the Primary Care Network to implement the medicines optimisation requirements of the new Network Contract, Direct Enhanced Service for enhanced health in care homes and other PCN medicines management responsibilities.
Main duties of the job
This is a training role which requires thesuccessful applicant to complete a CPPE course, therefore please ensure youunderstand the training requirements. These roles have been designed to beinteresting and fulfilling roles, providing a comprehensive range ofappropriate clinical pharmacy services to general practice in Surrey Heath. Thepost holders will undertake resident/patient-facing activities, and associatedmedicines management support activities, to ensure effective and efficient useof medicines within the practices and across their registered populations.
The post holder will be supported tohelp develop the role and work to improve the care and outcomes for thepatients across Surrey Heath PCN. To play a pivotal role in the delivery ofhigh quality primary health care within Surrey Heath

Qualifications
Essential
1. GCSEs (C and above) or equivalent
2. Masters degree in pharmacy (MPharm) or equivalent.
3. Post-graduate clinical diploma level or equivalent
4. training/experience.
5. Have completed, be enrolled in, be undertaking or
6. prepared to undertake an approved primary care
7. pharmacy educational pathway (PCPEP), the
8. CPPE Primary care pharmacy education pathway or
9. Medicines optimisation in care homes training pathway
10. for pharmacy professionals.
Desirable
11. Willingness to undertake independent prescribing training.
Experience
Essential
12. In depth therapeutic and clinical knowledge and
13. understanding of the principles of evidencebased
14. healthcare.
15. An appreciation of the nature of GPs and general
16. practices.
17. An appreciation of the nature of primary care
18. prescribing, concepts of rational prescribing and
19. strategies for improving prescribing.
20. Excellent interpersonal, influencing and negotiating
21. skills.
22. Excellent written and verbal communication skills.
23. Experience of motivational interviewing and able to train

25. Demonstrate the ability to communicate complex and
