The Role:
As a registered Social Worker in the Hampshire County Council Continuing Healthcare (CHC) team, you’ll work within a multidisciplinary environment collaboratively with your social care colleagues and other professionals in the varied and complex field of NHS CHC. You’ll be supported to manage a diverse caseload, working across all care groups including older and younger adults, with mental health concerns and physical and learning disabilities.
What you’ll be doing:
* Supporting individuals with their onward care journey following a Discharge to Assess period from hospital.
* Undertaking assessments in line with the Care Act 2014.
* Engaging with multidisciplinary teams to support completion of NHS CHC assessments.
* Collaborating with individuals and their families to transition them to suitable care provision following assessment.
* Undertaking mental capacity assessments.
* Managing challenging and complex cases.
What we’re looking for:
* Social Work qualification and registered with Social Work England (SWE). We would welcome applicants wanting to complete their ASYE.
* UK based experience within Adult Social Care.
* Experience and/or knowledge of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care July 2022 (Revised).
* A sound understanding of how to apply a strengths-based approach.
* Understanding of social work issues, legal frameworks and social services’ statutory duties.

Travel Requirements:
You will be required to travel to various locations within Hampshire and it is essential that you have a current driver’s licence and access to a reliable vehicle on a regular basis.
