FULL TIME EXPERIENCED SOCIAL WORKERS
Newly developed Hospital Discharge Team & Community Home First Hub Service.
An exciting opportunity is now available for the correct Social Work candidates to join a hybrid social care services supporting patients in the acute setting and those patients placed to Discharge to Assess & Long-Term facilities.
This role is based over two facilities, one acute and one community which both support multidisciplinary working. As part of this multidisciplinary approach, you will work hand in hand with therapy, nursing & reablement teams all focused with the same goals of completing personalised holistic assessment to support patients home safely and without delay.
Here at the Northern Care Alliance, the social care teams work 37 hours weekly which covers 7 day working via a team rota which includes days/ evenings/weekends.
Responsibilities:
1. Be person centred and reinforce a patient’s choice and control.
2. Promote a patient’s well-being throughout their journey in the acute and community setting.
3. Provide Strength Based Assessments and independence.
4. Complete Care Act 2014 compliant assessment.
5. Source provisions both formal and informal in support of patient needs.
6. Interact with a plethora of professionals daily.
7. Guide and support colleagues from other professions or colleagues less experienced in social care processes.
8. Have knowledge of related legislation, policy, and frameworks.
9. Experience of Safeguarding processes and procedures.
10. Complete mental capacity assessment and best interest processes for patients unable to make specific decisions.
11. Knowledge of the Deprivation of Liberty Safeguards.
12. Be an active and integral part of the team.

In addition, we also recognise previous, continuous, national local government service regarding annual leave.
