What the Job Involves This role involves the provision of Leaving Care Support Services through the Pathway Assessment and Planning Process. Key responsibilities include supporting young individuals in areas such as education, training, employment, accommodation, health, personal support, family and social relationships, practical skills, and financial assistance. You will work closely with individual service users to offer support, advice, guidance, assistance, and counselling, ensuring adherence to the Council’s policies and procedures.
