Ravensworth Surgery hasan exciting opportunity for a registered Practice Nurse to join our team. We have a patient population of just shy of 6000patients spread across 2 surgeries in South Tyneside. We are a friendlypractice looking for an experienced practice nurse to work with post holder is responsible for the delivery of basic practice nursing services and care to the practice postholder will work collaboratively with other members of the general practice team in order to meet the needs of the patient, and may need to liaise with members of the wider community teams. JOB SUMMARY:
The post holder is responsible for the delivery of basicPractice nursing services and care to the Practice population. This includes (butis not limited to) immunisations, screening (including cervical screening),opportunistic consultations, chronic disease reviews, ear care, woundmanagement, health promotion and contraceptive advice/review. The postholder will work collaboratively withother members of the general practice team in order to meet the needs of thepatient, and may need to liaise with members of the wider community andhospital teams to ensure the delivery of holistic care to the patient. The postholder will initiate and manage amethodical assessment of patient needs including public health, healtheducation and promotion in consultation with the GP partners. They will alsoassist in the development of clinical protocols and guidelines in addition toaudit such as infection control. They will ensure the maintenance of equipment,storage of vaccinations and stock levels relating to patient care.

The post-holder will implement andlead on a full range of promotion and management their own and others healthand safety and infection control as defined in the practice Health & Safetypolicy, the practice Health & Safety manual, and the practice InfectionControl policy and published procedures. This will include (but will not belimited to):
1. Using personal security systems within the workplace according to practice guidelines
2. Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines
3. Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport
4. Management and maintenance of Personal Protective Equipment (PPE) for the practice including provision, ordering, availability and ongoing correct usage by staff
5. Responsible for hand hygiene across the practice
6. Ownership of infection control and clinically based patient care protocols, and implementation of those protocols across the practice
7. Active observation of current working practices across the practice in relation to infection control, cleanliness and related activities, ensuring that procedures are followed and weaknesses / training needs are identified, escalating issues as appropriate
8. Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process
9. Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes
10. Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand cleansing facilities, wipes etc are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate.
11. Safe management of sharps procedures including training, use, storage and disposal
12. Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and safe way, free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management
13. Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised
14. Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector managers
15. Undertaking periodic infection control training (minimum twice annually)
16. Routine management of own team / team areas, and maintenance of work space standards
17. Waste management including collection, handling, segregation, container management, storage and collection
