As a GP Link Worker you will provide crucial support to individuals facing various challenges. Working closely with community-based health and social care providers, you will aim to prevent crises by offering timely assistance and guidance to those in need. You will also be supporting those transitioning from hospital settings back into the community, focussing on promoting independent living and reducing the risk of readmission. Through proactive intervention and access to appropriate services, your goal is to empower people to overcome obstacles and improve their overall well-being.
A day in the life of a GP Link Worker will involve you:
1. Providing support and information to clients, aiding in the development of independent lifestyles through connection with supportive groups and activities.
2. Recording and reporting client activities, ensuring appropriate actions are taken based on changes in their condition or circumstances.
3. Liaising with primary care, social care, health, housing, and other professionals to ensure comprehensive support for clients.
4. Collaborating with clients to establish short, medium, and long-term goals, developing personalised care and support plans.
To be a successful GP Link Worker, you’ll need:
5. Demonstrates passion for achieving agreed outcomes and improving client well-being.
6. Works effectively under pressure, managing tasks with minimal supervision.
7. Strong team player, capable of collaborating within a multi-level team structure.
8. Demonstrates accountability and professionalism in interactions with service users, partners, and stakeholders.
