The clinical beds teamwork with patients in care homes where the clinical beds are located during their short stay, covering week days and weekends. Our clinical beds are based across 3 locations in Nottingham City. This post will cover all the 3 locations providing rehabilitation for patients in the clinical beds.
We are a busy, friendly, cohesive team, who would welcome the opportunity to work with a new member of staff willing to contribute to the on-going development of the services we provide. Our Assistant practitioners work closely to plan and implement treatment plans for patients along with Nurses, Occupational Therapist or Physiotherapist.
Previous experience working in a care or rehabilitation setting is highly desirable.
Main duties of the job
1. To support the work of the registered practitioners in the clinical beds team within the parameters CityCare Assistant Practitioner competencies.
2. Maintain excellent communication with patients, relatives and members of the team regarding all aspects of care, demonstrating a variety of communication skills in accordance with patient need.
3. Perform patient assessment (under supervision and after appropriate delegation from the registered practitioner). Assessments to include tissue viability, nutrition and falls.
4. Implement and evaluate individual care plans, utilising practical and theoretical knowledge under the supervision of a registered practitioner.
5. Undertake the daily review of patients health and social care needs as required against the care plan; take the appropriate action to achieve the planned aims.
6. To carry out environmental assessments and consider need for equipment, including ordering, fitting and ensuring safe usage.
7. Perform area specific clinical competencies to enhance the delivery of patient care as delegated by the registered practitioner.
8. To carry out rehabilitation programmes as agreed with a registered professional.

Job Purpose
9. To work alongside Nurses, Physiotherapists, Occupational therapists and rehabilitation Support Workers to provide assessment and treatment intervention to people within the Nottingham City Clinical beds setting.
10. To support the work of the clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values.
11. You will be working in a variety of care home settings where the clinical beds are located following treatment plans and setting up assessment and care plans for clients, carers and families.
12. You will participate actively in the services training strategy, including in-service training and multi-agency training.
Dimensions
13. The Assistant Practitioner will work as an active member of the Clinical Beds team in providing a high quality, efficient and effective service to patients.
14. The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction/supervision of a registered practitioner. They will work within relevant legal and ethical frameworks in accordance with organisational protocols and policies.
15. The Assistant Practitioner will be proficient, knowledgeable and competent. Educated to Foundation Degree level, or equivalent, they will continue to develop in their role through the acquisition of further skills, knowledge and competencies, to be determined by the needs of the patients, the service and the team.
Key Responsibilities
Clinical
16. To undertake individual treatment and interventions with clients under the supervision of qualified staff.
17. To refer service users to external agencies and specialists when their needs are beyond the assistant practitioner role (See guidelines)
18. To accept responsibility for designated caseload, working independently taking an active role in promoting independence and meeting the needs of the service user.
19. To promote wellbeing, safe mobility, transfers and effective communication. To provide instruction, advice and intervention to service users and carers in line with the multi disciplinary team guidelines, (see guidelines).
20. To assess, provide, fit and demonstrate use of a wide range of equipment to promote independence. (See guidelines).
21. To assess for and commission a range of adaptations in line with the multi disciplinary team guidelines (see guidelines).
22. To attend and contribute to team meetings and case discussions using effective communication.
23. To maintain comprehensive and accurate records in line with employers polices
24. To promote health and wellbeing with all citizens
25. To undertake basic wound care tasks as delegated by the District Nurse.
26. To undertake a range of nursing competencies, as per the AP competency framework.

27. To provide support to other Assistant Practitioners as appropriate.

28. To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames (as detailed in the competency guidelines).
29. To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Also to keep an active up to date CPD portfolio.
