Job summary
The objective ofthe Business and Transformation Manager is to provide a central source ofleadership and management to aid service transformation for our population.
The ideal candidatewill need to be proactive, have a passion for Primary Care, and have a proventrack record in delivering Primary Care improvements.
The remit of thisrole is broad and will require attention to detail in delivering high-qualityday-to-day operations.
You would supportincreased access to care for patients on a range of integrated digitaltransformation projects that would enable the staff to work more effectivelyand help to sustain the future of general practice services.
Main duties of the job
As Business andTransformation Manager you would work within the PCN to lead and develop astrategic plan to build a robust infrastructure for Horsham Central PCN, which:
1. Is underpinned by a quality improvementmethodology
2. Drives integrated working
3. Drives efficiency and reduces duplication
4. Reduces variation between practices
5. Is underpinned with easy-to-understand data packs to support decision-making thatincludes but is not limited to demand, capacity, utilisation data, staff, andpatient experience
6. Builds staff skills and capabilities
7. Enhances patient access whilst protecting ourworkforce
8. Takes into consideration the digital maturity ofthe PCN
9. Takes into consideration the population's healthneeds and health inequalities within our community

Alliance for BetterCare CIC is a GP Federation that unites 47 NHS GP practices across 12 PrimaryCare Networks in Sussex and Surrey. We support our Primary Care colleagues aswell as their patients, to transform how healthcare is managed within the community. Our PCN builds on the existing primary careservices and enables a greater provision of proactive, personalised and moreintegrated health and social care. We are supported by practitioners inadditional roles who allow us to create bespoke multi-disciplinary teams basedon the needs of our local population.
Job description
Job responsibilities
To work closelywith the Practice Managers in developing the PCNs use of new services,processes and technology to improve overall efficiency in a range of projectsincluding but not limited to, cloud telephony, digital triage, promoting theuse of the NHS App, and optimising the use of clinical systems.
Strategic Planning
11. To lead and develop a strategic plan thatincludes a robust digital infrastructure
12. Support PCNs in holding their own mission andplan for delivery, translating this to a business plan against which they cantrack progress
13. To clarify objectives and priorities for networkdelivery in line with operational capacity, financial parameters and contractrequirements
14. Promote the engagement of all PCN practices andact as an ambassador for the network
15. To proactively build relationships with systemleaders to help influence proactive and new models of care
16. To use business intelligence tools to aiddata-driven decisions
Business Operations
17. To provide oversight and accountability to thedelivery of PCN contracts and specifications
18. Identify and record risks and issues and developcontingency plans with Network Leads
19. Ensure all relevant national strategies,policies, targets, and frameworks are fully implemented
20. To drive issues through to resolution, acting asa point of expertise
21. To support the collation of financialinformation, compiling and presenting financial reports
22. To ensure that the PCN's physical and financialresources are managed and maintained for maximum effectiveness
23. Manage allocated staff, including recruitment,induction, setting workload priorities, delegating tasks, day-to-daymanagement, including appraisal/ investigations etc
24. Work toestablish teams of ARRS staff within PCNs, supporting PCNs to identifyworkforce need and develop or recruit staff in to this
25. EnsureARRS staff have all necessary support to thrive in their role includingobjective setting with the PCN lead and providing a regular touch point for anyconcerns
26. Support or establish where not already inexistence networks of ARRS roles of specific disciplines between PCNs
27. To foster a culture of collaboration and promotelearning to drive continuous improvement
28. To take an active role in the development andembedding of the PCN's culture, values and reputation
29. To represent the network at externalmeetings
30. To facilitate progression for the PCN staff topromote retention
Projectmanagement
31. Drive improvements in patient access, service,and experience
32. Ensure that digital and operationaltransformation is embedded and aligned with system and national strategies incollaboration with the ICB Digital Transformation team
33. Manage the set up and delivery of multipleprojects working with colleagues and experts to scope, plan, design and overseeeffective delivery
34. Provide expertise of best practice change andproject management methodologies
Analysis
35. Interrogate and analyse data to assess demand,capacity, and activity to support the delivery of clear project reporting andproject outcomes
36. Submit regular high-level project reports showingprogress against milestones, status, resource requirements, risks anddependencies and recommend a range of options
37. Develop key performance indicators and milestonesfor specific programmes and projects, managing conflicts and ensuringinterdependencies are managed effectively
38. Create and maintain accurate documentation inrelation to finances, standard operating procedures, contracts, contacts, anddecision-making, which is accessible centrally
Serviceimprovements
39. Hit government set targets and support populationhealth management
40. Identify opportunities for streamlining theservice within the PCN
41. Enable PCN staff to maximise clinical efficiencyand enhance internal and external communications

44. Communicate the vision for the PCN and supportthe development of strategy and operational policies to support this vision
