Job description
As a Band 6 Occupational Therapist, you will play a crucial role in delivering high-quality patient-centred care, contributing to the rehabilitation and well-being of individuals within our community. Responsibilities include, but are not limited to:
* Conducting comprehensive assessments of patients' functional abilities, identifying barriers to independence.
* Developing and implementing individualised treatment plans to address physical, cognitive, and emotional challenges.
* Collaborating with multidisciplinary teams to formulate and execute rehabilitation plans, promoting independence and optimising patients' quality of life.
* Fostering strong therapeutic relationships with patients, ensuring their involvement in goal-setting and treatment planning.
* Providing education and support to individuals and their families.
* Maintaining accurate and up-to-date patient records, ensuring compliance with relevant policies and standards.
* Generating clear and concise reports to communicate progress and recommendations to the wider healthcare team.
* Work collaboratively with other healthcare professionals, including physiotherapists, nurses, and social workers, to ensure coordinated and holistic care for patients.
Requirements:
* Occupational Therapy Degree.
* Current registration with the Health and Care Professions Council (HCPC).
* Minimum of two years of post-qualification experience in a relevant clinical setting.
* Proven experience in conducting assessments and delivering occupational therapy interventions.
* Excellent communication and interpersonal skills.
* Ability to work effectively within a multidisciplinary team.
* Commitment to professional development and evidence-based practice.
