Job summary
An opportunity has arisen at Friary House Surgery for a practice nurse with ideally, diabetes and/or respiratory experience, to join our friendly team of nurses who serve a community of 12700+ patients. This role is advertised as full time hours although there may be room for negotiation.
Main duties of the job
The successful applicant will be involved with a full range of treatment room duties and management of long-term conditions. This is an exciting proposition for the right candidate to work with a forward thinking and dynamic team. The successful candidate will provide assessment, treatment, screening, health education services and advice to patients on general health with support and guidance from our experienced team of 4 GPs, 6 Clinical Practitioners and nursing team.

In addition to the clinical and medical practitioners we are supported by a pharmacy team, counsellors, podiatry team, mental health team and physiotherapists and administrators and receptionists. Friary House Surgery also favours training and CPD opportunities.

The coreresponsibilities of the Practice Nurse are as follows. On occasion, there maybe a requirement to carry out tasks not listed below, that are within thereasonable scope of the role.
To supportthe Infection Prevention Control (IPC) lead and to work to the IPC Policysupporting the implementation of audits, training, support and evidence ofcompliance to the Practice/OperationsManager
To supportthe production of the IPC annual statement in conjunction with the Practice/OperationsManager
Todevelop, implement and embed an effective care management programme for thefrail and housebound patients registered at the practice
To assessthe needs of patients ensuring the provision of healthcare is appropriate,incorporating evidence=based practice
Develop,implement and embed health promotion and wellbeing programmes
Implementand evaluate individual treatment plans for chronic disease patients thatpromote health and wellbeing
Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health
Provideroutine nursing care to patients in accordance with clinical based evidence,NICE and NSF
Todiagnose and manage acute and chronic conditions as required
To reviewpatient medications to enhance compliance
Provideguidance to patients in the use of prescribed and over-the-counter medicationsregarding side effects and dosages
Co-ordinatethe provision of travel medicine services
Requestpathology services and process pathology results as required
To overseethe provision of chronic disease clinics, delivering patient care as necessary,referring patients to secondary/specialist care as required
Maintainaccurate clinical records in conjunction with existing legislation
Ensureread codes are used effectively by all member of the nursing team
Ensureappropriate use of the chronic disease registers
Todevelop, implement, embed and participate in the will woman and well manclinics
To ensurecompliance with the practice chaperone policy
To assistsGPs with minor surgery when required
Prioritisehealth issues and intervene appropriately
To supportthe team in dealing with clinical emergencies
Recognise,assess and refer patients presenting with mental health needs
Toimplement vaccination programmes for adults and children

To liaise withexternal services/agencies to ensure the patient is supported appropriately
Thedelegate clinical responsibilities appropriately, ensuring safe practice andthe task is within the scope of practice of the individual

Qualifications
Essential
2. Nursing degree or equivalent UK NMC Registered nurse
Desirable
3. Diabetes and/or respiratory.
4. Travel Medicine.
5. Hypertension.
6. Cervical cytology.
7. Child immunisation.
8. Phlebotomy.
Experience

9. Experience of working with a nursing team and of dealing with vulnerable patients.
