Job summary
This post is an exciting opportunity for an experienced practitioner to lead a team focused on improving health outcomes for people with a Learning Disability across the healthcare system.
The purpose of this role is to support the learning from lives and deaths (LeDeR) national programme, local key themes and trends, supporting the service specification of ensuring that learning disability is "everyone's business".
The role will lead a team focused on the development and delivery of service improvements resulting from key themes from LeDeR, as well as other projects and training to promote inclusion and improved health outcomes for people with LD.
Where necessary you will be required to work at other bases or work remotely from home.
Main duties of the job
The role will co-ordinate and support the delivery of service improvements resulting from key themes from LeDeR, as well as other projects and training to improve the health outcomes for the learning disability population.
You will be responsible for providing leadership and direction to a team of practitioners, ensuring a project management approach to develop and achieve an annual programme of work; aligned to the system priorities; across the ALDH service and wider Sirona organisation.
You will act as a highly specialised source of subject matter expertise around reasonable adjustments for learning disability, providing a specialist resource hub for the ALDH service and to the wider health services within Sirona, developing and signposting resources.
You will be required to develop a health promotion programme for people with learning disabilities - linking in with relevant health services and existing structures, including the voluntary and community sector.
You will workwith service users, engagement and self-advocacy groups in order to champion the voice of people with a learning disability in the development of services.
