Following investment into the community hospital wards wehave an exciting opportunity to join our team. We are seeking a highlymotivated and enthusiastic Band 4 Therapy Practitioner to join our therapyrehabilitation team based at Chester le Street. The post would be ideal for anexperienced Band 3 therapy assistant or similar.
Applicants must be prepared to participate in a 6 day weekservice, covering bank holidays and Saturdays on a rota basis.
The post will be based at Chester le Street hospital;successful applicants will be based predominantly at one site but, will be requiredto work across Shotley Bridge and Weardale hosptials.

Main duties of the job
This is an exciting role with a great deal of patientcontact and opportunities to increase and consolidate both skills andknowledge. You will work under direct supervision of the senior Occupationaltherapists and Physiotherapists. You will have the autonomy to assess specifiedpatients as well as carry out rehabilitation and help facilitate timelydischarges and maximise patients' independence. You will act as a source toprovide health education and advice to patients, carers, and their families.
The post will combine competencies from physiotherapy andoccupational therapy disciplines, to support the delivery of the therapyservices to patients within a community rehabilaition ward setting.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
