Main duties of the job
* Providing a full range of nursing services, including chronic disease management, immunizations, and health assessments
* Collaborating with GPs and other healthcare professionals to deliver high-quality patient care
* Promoting health education and lifestyle advice to patients

Job responsibilities
* Deliver exceptional patient care with compassion and professionalism
* Conduct health assessments and provide advice on health-related issues
* Administer vaccinations and medications as prescribed
* Maintain accurate patient records and ensure confidentiality
* Collaborate with the healthcare team to ensure seamless patient care
* Support the Infection Prevention Control (IPC) lead and work to the IPC Policy, supporting the implementation of audits, training, and compliance evidence to the practice manager
* Provide routine nursing care to patients in accordance with clinical-based evidence, NICE, and the NSF as required
* Implement vaccination programmes for adults and children such as Flu, Covid, and Shingles
Person Specification
Experience
* Proven practice nurse experience within general practice.
* Experience in administering immunizations and vaccinations
* A patient-centred approach with a commitment to delivering high-quality care.
* Ability to manage time effectively and work independently
* Strong communication and interpersonal skills, with the ability to work effectively as part of a multidisciplinary team
* Experience of QOF-related activities such as cervical screening and baby immunisations
Qualifications
* Registered Nurse qualification with valid NMC registration
