1. To support and assess people who are in discharge to assess services. This includes recuperation and rehabilitation beds, recuperation and reablement services at home.
2. To complete carers assessments
3. To work in partnership with health colleagues to meet individual’s needs.
4. To be creative and work with individual’s strengths and community assets to meet their needs.
5. To carry out mental capacity assessments and complete best interests decisions.
6. To ensure that people are safeguarded and any risks are appropriately minimised.
7. Manage a case load and prioritise the competing demands and time pressures of ensuring there is continual flow through the discharge to assess services.
