We are an integrated team who work collaboratively with multiple professionals from local authority and health. We provide staff with regular supervision and hold a weekly meeting to discuss complex cases in a multidisciplinary arena.
Our service offers secondary mental health care for adults living with dementia and people diagnosed with mental ill health over the age of 65. Our team work closely with the Young Onset Dementia Service.
Through the provision of recovery focused care and treatment, we encourage employees to adopt a strengths based approach whilst striving to achieve personal outcomes for individuals. Our team work closely with Citizens, families and carers to ensure they are safe and live well in their communities.
About the role
Pay Details: Grade 8 - 9, SCP 26 – 30 £34,834 - £38,223 p.a. - SCP 31 - £39,186 - £43,421
Working Pattern: Mon- Fri
Main Place of Work: Llanfair Unit, Llandough Hospital
To assess Adults to determine eligibility for the provision of services, promote user empowerment and independence. To offer appropriate information advice and signposting to alternative services to meet identified needs.
About you
You will need:
1. Experience of working with adults who are vulnerable through illness, disability or age related frailty
2. Assessment of Adult service users and carers.
3. Understanding of illness, disability and age related frailty and its effects on individuals/carers/family
4. Knowledge of relevant legislation eg Social Services and Well-being (Wales) Act 2014, Mental Health Measure (Wales) 2010, Mental Capacity Act 2005 and Mental Health Act 1983.
5. Knowledge of Safeguarding issues and procedures in relation to Adults
6. A good standard of general education.
7. A recognised Degree/Diploma or equivalent qualification in Social Work i.e. Degree, DipSW, CQSW etc.
8. Registration with Social Care Wales
