Connect acts as a bridge between the local Integrated Neighbourhood team, working collaboratively with our cross sector partners to improve health and wellbeing of our population by using data and community insights to:
1. Prioritise prevention
2. Drive integrated working arrangements
3. Optimise the use of exiting resources
4. Proactively support people at the greatest risk of the poorest outcomes
5. Build resilient communities
We have two posts, working in East Suffolk Council and Babergh and Mid Suffolk District Council boundary. The posts involve hybrid working based at home, attending the office base and travelling around the area to meet colleagues and partners.
Main duties of the job
The post holderwill support the Connect Integration Partnerships Manager in delivering thefollowing areas:
Ensureeffective delivery of the portfolio of work associated with INT, Alliance andIntegrated Care Board priorities and programmes
Workclosely with a wide range of internal and external stakeholders includingrelevant patient, service user groups and communities to ensure transparent andhighly effective consultation and involvement processes result in an informedapproach to self care and self management initiatives
Tosupport and contribute to strategy development, interpretation andimplementation of policy in relation to health and wellbeing and to ensure thisis timely and pertinent
Developand manage the teams work programme and associated risk and issue managementas directed by the Connect Integration Partnerships Manager
Proactivelyengage with partners to share best practice and support roll out where require.
Ensuringjoined up working between East Suffolk Council and Alliance colleagues;housing, health and wellbeing strategies, priorities and INT development plansand encourage self care and self management initiatives.
Theoversight and management of resources and programmes of work associated withINTs and the Alliance.
Coordination of the local Connect meetings in accordance with the PartnershipsConnect and INT work programme and relevant system Programme Boards
About us

The post holderhas a pivotal role within the busy Partnerships team, which will includesupporting the Connect Integration Partnerships Manager to build and utiliserelationships with key partners, who are represented at the IntegratedNeighbourhood Team (INT) to foster a collaborative approach to deliverintegrated models of care and service provision for local people within theircommunity.
The post holderwill work as part of a dynamic group including service users, patients,clinicians, stakeholders, VCFSE, Anchor Institutions and statutory services tosupport the delivery of the objectives and outcomes of our Alliance and INTimprovement programmes, East Suffolk Health and Wellbeing Strategy and projectswithin our Connect areas within East Suffolk Council boundary.
The post holder will:
Supportthe Partnerships and wider Connect teams to ensure that the INT and Alliance programmesof work are planned and carried out effectively.
Workclosely with the local community and community representatives to understandthe challenges and issues that face the populations in each of the three INTareas (Saxmundham, Woodbridge and Felixstowe).To ensure that local health and wellbeing and care needs inform both thestrategy development and service delivery including the changing needs of thelocal population.
Supportthe Connect Integration Partnerships Manager in cross partner development whichstimulates and further progresses system wide changes through an integratedapproach.
Developand understanding integrated arrangements that enable cross boundary workingwith INTs and Connect
Workas directed with relevant partners, including provider organisations, publicsector agencies, GP practice staff, VCFSE organisations, community groups, faithgroups, Anchor Institutions, patients and members of the public, Healthwatch,Patient Participation Groups, Town and Parish Councils to progress system widechanges and improvements through an integrated approach built on INTs and widerConnect model.

Workwith the Connect Integration Partnerships Manager to deliver a local programmeof work within East Suffolk INTs (including the use of Population HealthManagement) which will:

oBeinformed by the health and care needs of the local population
oMeetall statutory, regulatory and NHS requirements ensuring alignment with theactivity of the organisation
