We are looking for a compassionate, enthusiastic and caring Health Link Worker to join our award winning Homeless Outreach Nursing you have good interpersonal and organisational skills, non judgemental and enjoy a challenge and have a passion to deliver a high standard of patient care in a variety of settings, then this is the opportunity for you!
Main duties of the job
To work as part of a team to provide early intervention, prevention, outreach nursing support for people who are experiencing homeless or rough sleeping across Somerset. This includes hostels and supportive/temporary or vulnerable accommodation.
This post is to cover Sedgemoor, however you may be required to cover other areas.

Qualifications
Essential
1. Level 2 English and Maths (equivalent to GCSE grade 4 or above).
2. An accredited level 5 vocational qualification such as a Foundation Degree or other Level 5 Diploma in Healthcare
3. Vocational Skills, NVQ Level 3 or equivalent in Clinical Healthcare, Support and/or Health and Social Care.
