Work Shift: Float Pool_5_Day (United States of America)
The Registered Professional Nurse assesses, coordinates, plans, directs, implements and manages the needs of assigned patients throughout the episode of care with a focus on progress toward discharge, including during transitions within the acute care stay. The Registered Professional Nurse is responsible for the delivery and coordination of patient care provided by the caregiving team. S/he assures that competent, compassionate patient care is uniformly provided to customers including referring facilities, hospital staff and patients in the inpatient, ambulatory, and community settings.
Essential Duties & Responsibilities, including but not limited to:

2. Assessment:
1. Collects pertinent data relative to the patient’s health or situation.
2. Integrates knowledge of evidence-based assessment parameters in care area.
3. Performs the skills of observation, auscultation, and palpation.
4. Prioritizes objective and subjective assessment data.
5. Focuses re-assessment according to the specific needs or condition of the patient.
3. Diagnosis and Planning:
1. Analyzes assessment data to identify patient problems, formulate expected outcomes, and determine strategies for meeting the patient’s individualized needs.
2. Applies knowledge of disease processes to formulate appropriate nursing diagnoses.
3. Documents problems, outcomes, and planning of interventions using standardized terms and technology.
4. Collaborates with the patient, family, and interprofessional team to formulate an individualized plan of care.
5. Demonstrates time management by prioritizing care to meet patient and family needs.
4. Implementation of the Nursing Plan of Care:
1. Implements strategies to address health promotion, prevention of illness/injury, facilitation of healing, alleviation of pain/suffering.
2. Performs/delegates interventions according to the Nurse Practice Act and AMC policy/procedure.
3. Works to develop technical skills, safe patient handling, and infection control techniques.
4. Coordinates care with the patient, family, interprofessional team, and community-based resources.
5. Delivers patient/family education with respect to culture, values, developmental level, and readiness for learning.
5. Evaluation:
1. Evaluates patient progress toward attainment of goals and outcomes.
2. Determines the effectiveness of interventions and strategies based on patient response.
3. Uses ongoing assessment data to revise diagnoses, outcomes, plan, and implementation strategies.
4. Re-assesses structures, processes, and timelines prescribed in the plan.
5. Documents significant cues of patient improvement or deterioration.
6. Education and Leadership:
1. Practices ethically.
2. Collaborates with consumers of healthcare and other caregivers in the conduct of care.
3. Serves as a charge nurse for the unit/department.
4. Seeks knowledge and competence that reflects current nursing practice.
5. Integrates evidence and research findings into practice.
6. Contributes to quality nursing practice through participation in unit/hospital committees.
7. Serves as a preceptor and competency reviewer.
8. Utilizes appropriate resources to plan, provide, and sustain evidence-based nursing services.
9. Practices in an environmentally safe and healthy manner.
