As a skilled Care Coordinator, youll already be used tomanaging a caseload of care packages and understand the varied nature andfast-pace of the role, along with all elements of associated care delivery.
Youll apply your gained communication and relationshipbuilding skills to effectively manage rotas for your care teams, respondefficiently to any day-to-day changes and act as the first point of contact forservice users, their families and other healthcare professionals.
No matter which care setting youve obtained your CareCoordination experience in, youll be aware that no two days are the same, soyoull already be able to evidence your adaptability and aptitude to work wellunder pressure, using your initiative and thinking on your feet.

This is an office-based role, Monday to Friday, 9am 5pmbut at times, travel across the East and South East region will be required.Therefore, applicants must hold a Driving Licence and have access to a vehicle.
You must also be committed to supporting the out of hourson call service, which is covered on a 1 in 5 rota basis across the team.
We're offering a competitive salary between £29,560-£34,560 per annum, which includes payment for is also theopportunity to earn bonus of up to £2,000pa.
