Job summary
Our Vision is to make every day the best day possible for patients, and those closest to them, living with life-limiting illnesses in South Devon. As Referral Co-ordinator for Community Services & Hospice at Home you will help to deliver this by being the first point of contact for all enquiries from patients and healthcare professionals, facilitating communication, responding to correspondence and processing referrals. You will provide efficient administration support to the Community Services (CNS, Social Work, OT, Physio) and the Hospice at Home Teams.
Main duties of the job
What youll bring:To be successful in this role, you will need to be compassionate and organised with excellent communication skills. You will be the first point of contact for patients, carers, and healthcare professionals, facilitating communication, processing referrals, and providing essential administrative support to our Community Services and Hospice at Home teams. Your ability to efficiently and compassionately manage a high volume of sensitive and urgent enquiries, while maintaining accuracy and attention to detail, will be key. You will be driven by our cause and will enjoy working in a fast-paced environment as part of a close team. To deliver on our strategy takes real teamwork, energy, and passionno two days are ever the same!
