Job summary
The Lincolnshire Mental Health, Dementia, Learning Disability and Autism (MHDLDA) Alliance has identified that to deliver on our ambitions of people living well in their communities, that a more strategic approach is needed surrounding housing, and that partnerships across the Lincolnshire Integrated Care System (ICS) need to be developed and enhanced to meet this need.
As part of the MHDLDA Strategic Development, Innovation and Integration Hub, this role will seek to address mental health and housing needs and work with partners to integrate services and strategies across housing, health, and social care.
We are seeking a strong, experienced leader who has specific housing expertise, to work within the Lincolnshire ICS to drive forward the agenda identified by the Lincolnshire joint accommodation strategy group and to the lead the development of a housing strategy, the associated workstream and plan.
You will have experience of working with a wide range of stakeholders, encourage collaboration within and outside of LPFT, and identify opportunities for future collaboration and transformation; aligning with the priorities identified by the MHDLDA alliance.

Main duties of the job
Lead the development of a housing strategy and associated workstream and plan.
Lead Lincolnshire Partnership Foundation Trust's (LPFT) work to ensure that the trust is optimising patient discharges as they relate to housing and that there is reduction in the number of patients without appropriate housing, including reducing delayed discharges from hospital and people placed out of area for whom accommodation is a barrier in returning to Lincolnshire.
Be a key and active member of the Lincolnshire joint accommodation strategy group
Provide detailed and comprehensive reports to the MHDLDA Joint Delivery group outlining delivery, progress and risks.
Lead the strategic engagement with partners across the Lincolnshire Integrated Care System to develop strong partnership working and deliver on greater integration pertaining to housing.
Plan and facilitate group discussions and workshops which lead to constructive outcomes
Demonstrates the ability to persuade and negotiate when conflicting views need to be brought to a consensus conclusion to achieve appropriate objectives
Be the Lead Professional for Housing and any future housing support team, ensuring that professional standards are defined and met, continuing professional development is available, and additional supervision is available for more complex areas of practice.

Job responsibilities
Lead the development of the Lincolnshire MHDLDA Alliance Housing Workstream, including the development of a housing strategy
Be a key member of the joint accommodation strategy group and play a key role in supporting the activities of the sourcing group.
Lead the development and delivery of the strategic objectives for the above.
Create and lead forums for housing development initiatives, taking direction and steer from the joint accommodations strategy group and developing a shared programme of work
Provide reports to the Mental Health and Dementia Joint Delivery Group on the workstream delivery, progress and key risks.
Lead the strategic engagement with external partners to develop the required partnerships needed to deliver greater integration of mental health and housing services and strategies. For example identification of housing need as well as developing a set of trusted landlords
Lead the Trusts work on optimising patient discharge solutions as they relate to housing by addressing key housing issues, including:
1. Identifying existing barriers to discharge across the Trust.
2. Educating/upskilling colleagues in Duty to Refer and homelessness reduction act obligations of Local Authorities.
3. To try to prevent LPFT ever needing to discharge people as NFA.
4. Identifying and working with partners to design solutions that are fit for peoples needs.
5. Develop a range of long-term settled and short-term transitional accommodation options that enable discharge at the most appropriate time, into the most appropriate accommodation depending on an individuals care/support/housing needs and vulnerabilities.
Be the Lead Professional for Housing and any future housing support team, ensuring that professional standards are defined and met, continuing professional development is available, and additional supervision is available for more complex areas of practice.
Represent housing issues at relevant trust-wide and system wide forums.
Promote the importance and impact of housing more widely/externally to all partners across Lincolnshire.
Reduce dependency on Residential and Nursing Care
To raise awareness of the impact of housing on our patients and the impact it has on NHS and Local Authority services ( reduced demand on services by keeping somebody well at home for longer).
Build relationships and partnerships with housing providers whose values align with those of the MHDLDA Alliance to develop new innovative housing options.
Ensure that Lincolnshire is at the forefront of good practice development, at local, regional and national levels.
Create opportunities for research and evaluation activity within the housing workstream, working with colleagues in the trust and external partners.

Qualifications
Essential
6. Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area
7. Post-graduate degree in Management Studies or similar qualification/comparable experience
8. Charted membership of the Charted Institute of Housing (CIHCM)
Desirable
9. Qualification or equivalent experience in Programme and Project Management methodologies such as PRINCE2 (Practitioner level)

10. Demonstrated experience of leading organisation wide change programmes and projects in complex and challenging environments
11. Experience of working with senior stakeholders, managing their expectations and reporting progress on Programme progress
12. Demonstrated experience and understanding of housing systems, services and provision, including the responsibilities of local government and the wider public sector
13. Understanding of the public sector and importance of undertaking a system wide approach to improving health.
14. Demonstrated experience in a Housing or Healthcare environment working at the interface between sectors and organisations
15. Experience of creating and giving presentations to a varied group of internal and external stakeholders at all levels
16. Experience of creating successful partnerships and managing stakeholder relationships
17. Experience of setting up and implementing internal processes and procedures
18. Knowledge of Financial Systems and experience of procuring services and delivering value for money
