Job summary
Working closely with multi-disciplinary teams across health, social and voluntary sectors youll deliver high quality, individualised,evidence basedcare to all patients, maintain confidentiality, privacy and dignity of care for patients and their families in the way that you believe is best, delivering our high service standards, sharing best practice, and actively improving the way we work.
As an OccupationalTherapistyou will work within the Intermediate Care Team, where you willwork collaboratively with ourteam of Physiotherapists, OTs,Nurses and Rehab Assistants, to deliver high quality care to patients in the community by assessing their needs and implementing a treatment plan to encourage their independent living and prevent hospital admissions.
Main duties of the job
Within this role, you will:
1. Deliveryof high quality, individualised, evidenced based care to all patients
2. Teaching of patients, carers and peers
3. Ensuring that therapy input is integrated into the patients overall care plan
4. Operating as an autonomous practitioner working without direct supervision and at times lone working
5. Providing support to unqualified staff and students
6. Working with managers to develop the service in line with patient need, local and national developments
7. To maintain accurate records in line with local standards and The Royal college of Occupational Therapists standards.
8. Maintaining confidentiality, privacy and dignity of patients
9. Learning, developing and adapting to change evidenced in CPD
10. Operating in a team and contributing to team decisions
11. Managing own caseload and prioritising work
